    Case Worker – Bilingual (Spanish/English)

    Status: Full-Time, Non-Exempt

    Salary: $24.73 per hour

     

    Schedule: 35 hours per week, Monday – Friday, 8:30 AM – 4:30 PM

    Position Summary

    We are seeking a compassionate and skilled Case Worker – Bilingual (Spanish/English) to support older adults across several sites (Kew Gardens, Pomonok, Jamaica, and Rego Park). This role focuses on helping older adults access vital services and build community connections through case assistance, benefits counseling, application support, information and referrals, telephone reassurance calls, and participant engagement activities.

    Key Responsibilities

    + Conduct client intake assessments to identify challenges such as personal and family adjustments, financial barriers, or physical and mental health needs.

    + Provide accurate information about available services, benefits, and entitlements, and assist with applications and recertifications.

    + Conduct on-site visits with new participants to promote engagement and participation in center programs.

    + Develop and maintain detailed case notes and care plans, including action steps and follow-ups.

    + Collaborate with program directors to ensure culturally sensitive services and effective use of community resources.

    + Build and maintain partnerships with external agencies to expand resources and advocate on behalf of participants.

    + Participate in meetings, training, and professional development opportunities.

    + Perform additional duties as needed to support staff and participants at older adult centers.

    Qualifications

    + Bachelor’s degree in Social Work, Sociology, Human Services, Psychology, or a related field preferred.

    + Minimum of two (2) years of experience working with older adults or providing case management services.

    + Knowledge of benefits/entitlements and community resources strongly preferred.

    + Excellent written and verbal communication skills, including the ability to communicate effectively with diverse populations.

    + Strong interpersonal skills and the ability to build and maintain positive relationships.

    + Self-motivated, resourceful, and able to work independently as well as collaboratively.

    + Ability to maintain confidentiality and handle sensitive information with discretion.

    + Proficiency in Microsoft Office (Word, Excel, Outlook) and database management tools.

    + Bilingual fluency in Spanish and English (verbal and written) required.
