    JOB SUMMARY:

    Provides supervision and direction to Parents as Teachers Health Educators, monitor case loads and coordinates. Assists Parents as Teachers Health Educators in using the PAT Foundational Curriculum to plan and deliver home visits to qualifying families in Salt Lake County. Ensures compliance with Parents as Teachers Essential Requirements as well as the Office of Home Visiting DHHS Home Visiting Program funding and grant requirements. Develops and maintains strategic community partners to promote program services and cross collaboration including resources and referrals. Supervises assigned staff including hiring, training, assigning cases, monitoring caseloads and annual performance evaluations.

    MINIMUM QUALIFICATIONS:

    Bachelor's degree from an accredited college or university in Social Sciences, Behavioral Sciences, Public Administration, or other closely related field; OR an equivalent combination of related education and experience.

     

    Due to the nature of this position, the successful applicant must pass a required background check through fingerprinting and enrollment in the continuous RAP Back (Record of Arrest and Prosecution Back) program in accordance with current County Human Resources policy requirements.

     

    Due to the nature of this role, vaccination or proof of vaccination may be required.

     

    Employee will need to obtain their certificate as a Parents as Teacher curriculum.

    ESSENTIAL FUNCTIONS

    + Provides supervision and direction to case management staff including answering questions and resolving problems within the work unit.

    + Creates a process for, and monitors, the assignment of caseloads to ensure appropriate supervision

    + Supervises staff, which includes hiring, orienting, training, assigning and reviewing work performance, annual work performance appraisals, and discipline.

    + Ensures that case managers perform intake, risk/need assessments, investigations, maintain up-to-date monitoring of program requirements, and provide appropriate referrals to outside

    + Reviews work performance, monitors accuracy and timeliness of case management activities, and conducts quality assurance audits.

    + Participates in the interviewing and selection of staff within the work Provides training, coaching, and ongoing support for new and existing case managers.

    + Schedules staff coverage to accommodate client services and workload needs by rearranging staff assignments as needed.

    + Serves as a liaison/information source between the work unit, community partners, treatment agencies, and the public.

    + Assists in the developing, writing, and implementation of internal policies, job descriptions, and guidelines in conjunction with Administration.

    + Provides coverage in the absence of the Program Manager or other supervisory staff to assist in the smooth operation of the unit/division

    KNOWLEDGE, SKILLS AND ABILITIES (KSA)

    Knowledge of:

    + Basic supervisory principles

    + Case Management methods and techniques

    + Community Social Service Agencies

    + Basic PC operations

    + Rules, practices, and procedures pertaining to the work unit to which assigned

    + Effectively interpret general policies, criteria, and procedures for specific situations

    + Understand county and division personnel policies and procedures

    + Plan, assign, and supervise the work of subordinates

    + Work as part of a team

    + Interact confidently with other service providers

    + Maintain complex and confidential records

    + Identify and understand problems and determine the appropriate measures to solve those problems

     

    Additional Information

     

    This position is community based and requires travel in your personal vehicle to client’s homes and community partners in Salt Lake County.

     

    Schedule is flexible to meet the needs of the program and families, some evening and weekend hours.
