    Job Description

    The LPN /GPN is responsible for passing medications and performing medical treatment as a staff nurse. The LPN / GPN may be responsible for directing and supervising the nursing care of residents on assigned unit. Provides appropriate evaluation of resident condition/and documents same, communicating vital information to appropriate personnel. The LPN / GPN ensures the delivery of nursing care on the unit is consistent with facility standards, and established policies and procedures. The GPN must work in conjunction with a Registered Nurse who assumes responsibility for the performance of the GPN until licensure. Will act as facility supervisor as assigned.

    DUTIES/RESPONSIBILITIES:

    • Supports and promotes the nursing department functions on the unit by following established policies and procedures.

    • Direct resident care duties.

    • Passes medication and performs treatments following facility protocol.

    • Evaluates resident care on unit and shift by direct observation, personal interviews and completion of quality improvement audit tools. (LPN)

    • This job description is not intended to be all-inclusive. The LPN /GPN will perform other duties as necessary.

    EDUCATION AND EXPERIENCE

    • Graduate of recognized accredited school of practical nursing and eligible to apply for licensure.

    • Applied for and receives a Limited Permit to Practice within 90 days in accordance with guidelines of professional licensing division. Maintenance of licensure requirements required. Works directly under the supervision of an assigned RN.

    • The Limited Permit is valid for:

    • one (1) year; or

    • 10 days after nursing exam if fail

    • Therefore, must pass the exam within one (1) year. However, anyone who takes the exam and fails prior to the year point must report this to the Director of Nursing and HR as the permit expires within 10 days.
