    Position Summary

    The Maternal, Child, and Family team in Lincoln County's Health and Human Services department is looking for the right candidate to take on the role of Program Support Coordinator. This position provides a wide array of clerical and administrative program support, and internal and external coordination with and for all Maternal Child and Family Health (MCFH) Programs.

     

    This is a very important role for us, which also includes aiding in the coordination of programs across MCFH section of Public Health. Building knowledge of MCFH program in the community through navigation, outreach, social support, and advocacy. Supporting day to day operations while providing valuable administrative support to leadership for advisory boards and other community engagement events.

     

    $25.32 - $34.04 per hour*

    Essential Functions/Major Responsibilities

    General Office:

    + Perform general office and clerical tasks.

    + Perform data entry, word processing, and spreadsheet functions.

    + Input data, prepare letters, reports, and correspondence.

    + Manage the office supply inventory and program incentive inventory.

    + Perform basic office administrative tasks to support the department.

    + Receive and process section mail and deliveries.

    + Serve as Program Support Coordinator and file clerk for the department.

    + Manage multi line phone system. Answer and transfer calls. Take messages and relay to appropriate staff.

    Management Support:

    + Prepare agendas, minutes, meeting materials for team meetings, community advisory board, and community engagement events.

    + Act as support to outreach and community engagement events as well as team meetings as needed.

    + Work with management team to update internal policies and procures and program support coordinator workflows as needed.

    + Attend and participate in team meetings, trainings, and activities to further the goals of the MCFH programs.

    + Aid in the management team in community outreach and education.

    + Support monthly reporting and data management for various MCFH programs.

    Client Records:

    + Maintain records and files.

    + Access and manage documents in multiple databases and electronic medical records.

    + Contact agencies for records.

    + Conduct chart review as needed to support the home visiting staff.

    + Use electronic medical records to enter, edit, and retrieve client information.

    + Use multiples databases to input data and run reports.

    + Verify client insurance information.

    Referral Coordination:

    + Manage daily program referrals.

    + Conduct outreach to new referrals within specified timeframe.

    + Communicate with referring providers as to referral status and to close the loop.

    + Communicate with staff and management team regarding referral status.

    + Depending on nurse caseload and program eligibility follow guidance from MCFH management to assign referrals as appropriate.

    + Educate prospective clients and community partners regarding programs and eligibility requirements.

    + Serve as client outreach navigator.

    + Communicate professionally with staff and the public.

    + Interact and convene with community partners and resources as necessary to assure population needs are being met.

    Other Functions:

    + Interacts with community partners and resources as necessary to educate and bolster referrals to MCFH programs.

    + Conduct bedside outreach to new parents at the hospitals for multiple programs as needed.

    + As applicable, utilizing relevant certifications in support of MCFH program components and staff.

    Typical Qualifications

    Minimum Education Level

     

    High School Diploma / GED

     

    Minimum Experience Level

     

    Two- Three years in referral coordination within a governmental or private non-profit health care system.

     

    Oregon Driver's License required within 30 days of hire.

    Desirable Qualifications:

    Any training or courses that supplement education in typing and computer entry. An associate degree and experience working within a governmental or private non-profit health care system is preferred. Experience working with Public Health and or MCFH specifically preferred. Bilingual and/or bicultural competencies for the Hispanic population are preferred. Relevant certifications that are able to support MCFH programs.

    Knowledge of:

    + Experienced knowledge of home visiting and MCFH programs, program eligibility, dual enrollment ability, and program coordination for clients.

    + A working knowledge of all MCFH programs and eligibility requirements.

    + Thorough knowledge of administrative office practices, spelling, composition, and mathematical calculation.

    + Working knowledge of standard reporting practices.

    + Administrative and clerical procedures and systems such as word processing, managing files and records, record keeping and reporting, designing forms, and other office procedures and terminology.

    + Insurance verification training and working knowledge of Medicaid and Medicaid eligibility requirements.

    Lincoln County offers a comprehensive benefit package including vacation, holiday, and sick leave as well as medical, dental, vision which is paid at 90% by the County and 10% by the employee. Other fully paid benefits include life, AD&D, long-term disability insurance. Other optional insurances are available. A 401K retirement plan is also provided. The County's current contribution rate is 11%.

     

    Regular and Trial Service employees working between 20 - 39 hours per week on a regular basis are eligible for pro-rated employee benefits.
