    Job Description :

    GENERAL OVERVIEW:

    This position will be responsible for coordinating quality management activities for the Positive Health Clinic HIV/AIDS program and related research activities and projects related to reducing health disparities that are sponsored by the Positive Health Clinic. Activities will include gathering and analyzing data, preparation of reports and facilitation of project teams for areas of improvement identified. The individual will work closely with HIV care providers and related staff regarding quality of care delivered to HIV infected and affected, collaborating with medical staff to monitor patient outcome, set clinical priorities, and recommend and implement professional improvements. The position is responsible for ensuring that quality management guidelines for Ryan White Part C and Patient Centered Medical Home are met and patient standard of care protocols adopted by the Positive Health Clinic are monitored for compliance.

    ESSENTIAL RESPONSIBILITIES:

    + Organize performance improvement program for Ryan White Part C funded projects, under the direction of the Medical Director and Administrative Director.

    + Develop work plan for quality monitoring activities for Part C to include various components of care: clinical, education, patient satisfaction.

    + Track progress, update work plan, and report status of activities to leadership group on a monthly basis. Collect and present accurate and timely data to display the results of quality improvement efforts.

    + Through continual assessment of internal and external environments, contribute to the establishment and prioritization of quality improvement activities.

    + Consult with essential stakeholders as appropriate to finalize and advance project goals. Develop and monitor indicators set to meet departmental quality improvement goals and improve quality.

    + Lead teams through effective use of quality and project management methods.

    + Delegate work appropriately and follow-up to provide guidance and oversight to project completion. Coach and advise team members as needed to fulfill their role in the project team.

    + Monitor and communicate regulatory changes and develop strategies based on these changes. Assure compliance with all guidelines and regulations related to implementation of quality management programs in Ryan White funded grantees.

    + Promote interdisciplinary partnerships and consult with hospital staff in activities relating to quality improvement practices. Establish and maintain positive working relationships with staff, medical staff leaders, and other support areas.

    QUALIFICATIONS:

    Minimum

    + Bachelor's degree in quality management, health informatics, nursing, or related field required or relevant experience and/or education as determined by the company in lieu of bachelor's degree.

    + A high level of analytical skill is required for this position. The individual will need to be familiar with analyzing statistical reports and articulating findings to diverse groups of health professionals.

    Preferred

    + Master's degree in public health or related field preferred. Prior experience in quality improvement within the healthcare industry preferred.
