+ The Community Health Educator will implement the Community Health department’s education and outreach plan, connecting and engaging with patients and community members through health and wellness events, collaborating with community partners and agencies, and direct outreach.
+ This role is also responsible for assisting patients and community members to connect with community resources and referral appointments, overcome barriers to care and navigate the health care system, including community agencies and organizations.
+ Lead the implementation of systemwide Community Health initiatives and oversee deployment of programs and community engagement activities within the assigned medical center region.
+ Develop and deliver health education by researching, creating, and implementing evidence-based curriculum and materials to promote awareness and access to health services across the lifespan.
+ Plan and manage community health programming, including organizing, implementing, and evaluating culturally sensitive education programs and presentations aligned with regional health priorities and community interests and needs.
+ Ensure accessibility of materials by tailoring programs to appropriate literacy levels and facilitating translation into Spanish or other languages as needed.
+ Conduct direct community outreach to priority populations, serving as a liaison between community members, partner organizations, and AHS services in alignment with AHS initiatives.
+ Support care navigation by helping individuals access healthcare services, including finding providers, using tools such as MyChart and Unite Us, and connecting with AHS resources like the physician referral line.
+ Assist patients with social needs by meeting in clinical and community settings to support health goals, including help with insurance, prescriptions, appointments, and referrals to resources such as food, housing, and substance use services.
+ Coordinate and represent AHS at community events, managing logistics, volunteers, and dissemination of culturally appropriate health education and system materials including system initiatives (i.e. mammograms, lung cancer screenings, etc.).
+ Foster collaborative relationships by maintaining partnerships with community organizations, public health agencies, and coalitions focused on key health priorities and populations.
+ Manage administrative functions such as scheduling, maintaining stakeholder contact lists, tracking participation and program impact, preparing reports, maintaining records, performance improvement, quality assurance, and best practices.
+ Maintain professional standards by keeping certifications and licenses current, participating in continuing education opportunities, and supporting department goals and initiatives.
Education:
+ Bachelor’s degree in Public Health, Health Education, or a related health field required.
+ Master's degree with a concentration in Public Health, Health Education, or a related health field preferred.
Experience
+ Certified Health Education Specialist (M/CHES®) preferred.
+ 3-5 years of experience in public health, health education, or a related health field preferred.
+ Certified Tobacco Treatment Specialist (CTTS) a plus.
+ Association of Diabetes Care & Education Specialists (ACDES) training a plus.
+ Excellent oral and written communication skills.
+ Strong organizational skills and ability to multitask effectively.
+ Experience with organizing, managing, and evaluating health and wellness initiatives.
+ Experience with developing and delivering programs.
+ Ability to function within an interdisciplinary team.
+ Ability to navigate EHR and databases.
+ Proficient in Microsoft Office applications.
+ Bilingual in Spanish and Creole a plus.
+ Valid NJ Driver's License.
+ Evening and weekend availability required.

Hackettstown Medical Center has been providing care to the community since 1973, primarily serving Warren, Sussex and Morris Counties in New Jersey.
