    Minimum Starting Salary: $62,000/annually Position Summary: The Behavioral Health Coordinator is responsible for fulfilling all contractual requirements regarding behavioral health services for enrollees in the Child Welfare Specialty Health Plan for Sunshine Health. This includes but is not limited to coordination of Multi-Disciplinary Staffings, monitoring Baker Act admissions and compliance with HEDIS measures associated with post-discharge activities, and providing clinical consultation and support for members with high acuity behavioral health needs. The Permanency Support Behavioral Health Coordinator position serves as a subject matter expert in the Child Welfare Specialty Plan benefits and requirements for children transitioning, and in, permanent placement to include reunification, adoption, permanent guardianship, and independent living. Technical/Functional Expectations: · Clinical Consultation and Care Coordination: serve as a liaison between Sunshine Health and case management, Operations, Adoptions, Post-Adoption and/or Independent Living to coordinate behavioral health care for members of the plan. Provides education regarding plan benefits and behavioral health care navigation to families, community partners, and other applicable stakeholders. · Multi‐Disciplinary Team (MDT): Coordinate and facilitate Multi-Disciplinary Team (MDT) meetings for any child or young adult on the Child Welfare Specialty Plan that experiences a Baker Act admission for the purpose of assessing the youth s needs and opportunities for interventions toward reduction of readmission. · Discharge Planning: Participation in discharge planning activities, following a youth or young adult s admission into an inpatient hospital or crisis stabilization unit, to coordinate services and ensure that the enrollee attends a seven (7) day follow up appointment. · HEDIS Care Gap Reports: Review of health and wellness reports (i.e., HEDIS Care Gap Reports), provided by Sunshine Health, which indicate services and performance measures that are due, or past due. Provide follow up and outreach as needed. · Quality Management: Review case management reports/data, available within the Sunshine Health Envolve Portal and respond to all requests for information from Sunshine Health. · Documentation: Ensure all care coordination activities are documented in compliance with Sunshine Health standards in the Sunshine Health Envolve Portal and FSFN. · Other duties as required: Job performance requires fulfilling other incidental or related duties as assigned, assisting, and training others, and performing duties of higher rated positions from time to time for developmental purposes. Business Experience: · Five years Clinical Experience: This is required in order to meet the unique behavioral health needs of children being served in the Florida s child welfare system of care. · Strength Based Family Centered Practice Orientation: This is essential in order to effectively support the System of Care and its values. · Strong Interpersonal, Presentation, and Organization skills: This is required in order to effectively partner with · various stakeholders and agency staff and adequately train community stakeholders. · 3‐5 years Child Welfare Experience: This is preferred in order to effectively understand that improving the well‐being of children in the child welfare system is community effort. · Valid Driver's License ‐ Essential: This is required in order to travel throughout the county and other locations. Educational and Experience Requirements: · Licensed or Licensure eligible preferred. · Master s in social work or MA/MS in Human Services field required. · 1 to 3 years of experience in child welfare preferred.
