Job Description Summary

We’re seeking a compassionate and organized Care Coordinator to serve as a vital link between hospital discharge and home health services. In this patient-centered role, you’ll ensure smooth transitions of care, provide exceptional customer service, and support patients in navigating their health journey from hospital to home. As a Care Coordinator, you’ll work closely with clinicians, care managers, and referral sources to coordinate services, close gaps in care, and help patients overcome barriers to achieving their health goals.

**Schedule:** Monday through Friday business hours.

**Location:** This position will be based at Utah Valley Hospital.

Key Responsibilities

+ **Coordinate Patient Referrals:** Manage and monitor referrals across the healthcare system to ensure timely scheduling and follow-up.

+ **Liaison Support:** Act as a bridge between patients, clinics, and care teams—resolving concerns, facilitating communication, and ensuring continuity of care.

+ **Transition of Care:** Guide patients through transitions from hospital to home health/hospice, ensuring they understand their care plans and available resources.

+ **Data-Driven Outreach:** Use analytics to identify care gaps and proactively engage patients to improve outcomes.

+ **Patient Advocacy:** Help patients overcome socioeconomic barriers by connecting them with appropriate community and home-based resources.

+ **Education & Promotion:** Serve as an ambassador for Home Services—educating referral sources on available services and maintaining up-to-date knowledge of community resources.

+ **Documentation & Communication:** Accurately document all interactions, collaborate with physicians, and care teams to develop patient-centered care plans.

Minimum Qualifications

+ Experience in a customer service role using enterprise software systems

+ Proficiency in Microsoft Office (Word, Excel, PowerPoint, Outlook)

+ Strong verbal, written, and interpersonal communication skills

+ Demonstrated problem-solving abilities and attention to detail

+ Ability to handle sensitive information with diplomacy and discretion

Preferred Qualifications

+ 1+ year of experience in home health or hospice

+ Familiarity with healthcare terminology, roles, and systems

+ Experience coaching or training others on enterprise or case management software

The hourly range for this position is listed below. Actual hourly rate dependent upon experience.
