Description:
Job Summary:
Oversees and coordinates the discharge planning and utilization management activities for assigned caseload. Facilitates appropriate utilization of resources and efficient patient progression through the continuum of health care resulting in the highest quality and most cost-effective care.

What you will need:
+ BSN; if ADN; BSN to be obtained within 4 years of hire
+ Current RN license in the State of Michigan.
+ Active BLS Certification
+ Preferred: Certification in Case Management or other related professional certification.
+ Two to five years of medical/surgery or chronic disease management experience in an acute care setting
+ Previous case management experience in emergency department and inpatient hospital preferred.
What you will work:
+ Full Time
+ 40 hours per week
+ 8a-4:30p
+ Working on different units including Med/Surg, ICU, and Psych Med
