Description We are looking for a detail-oriented Nursing Coordinator to join our team in New Hyde Park, New York. This Contract-to-permanent position offers an opportunity to play a vital role in coordinating nursing visits, managing patient care assignments, and ensuring seamless communication between healthcare teams. The ideal candidate will bring strong administrative skills and a commitment to supporting high-quality patient care.

Responsibilities:

• Coordinate and oversee regularly scheduled nursing visits to ensure timely and efficient patient care.

• Communicate new patient assignments and primary nursing responsibilities from the Nurse Manager to field staff.

• Track and secure necessary authorizations and reauthorizations for nursing visits as required.

• Provide infusion treatment plans to physicians and maintain accurate communication logs.

• Monitor monthly visits conducted by external agencies and maintain detailed records.

• Organize and maintain credential and insurance documentation files for outside agencies.

• Collaborate closely with Intake and Pharmacy teams to coordinate nursing visits and process patient orders effectively. Requirements • Proven experience in administrative coordination, preferably in a healthcare setting.

• Familiarity with home health operations and processes.

• Strong ability to answer inbound calls professionally and manage communication effectively.

• Proficiency in managing calendars and scheduling appointments.

• Excellent organizational skills with attention to detail.

• Ability to multitask and work collaboratively with diverse teams.

• Knowledge of insurance documentation and credentialing processes is preferred. TalentMatch®
