THE POSITION

DESCRIPTION OF WORK
As an Aging Care Manger 2, you will be responsible for answering calls at the Senior Line Call Center and meeting with clients, in-person or virtual. You will have the opportunity to identify and confirm client needs and provide relevant and accurate information on accessing appropriate resources and programs. You will be expected to explain the Intake/Assessment/Care Management process for relevant programs, conduct intake interviews and complete intakes for in-home services. Timeliness is key when responding to messages and requests for printed information, brochures, and applications . You will be responsible for completing required forms and documentation for submission to the appropriate parties while ensuring all aspects of your work are accomplished accurately and in a timely manner.
Work Schedule and Additional Information:
+ Full-time employment
+ Work hours are 8:30 AM to 4:30 PM, Monday - Friday, with 60-minute lunch.
+ Telework: You may have the opportunity to work from home (telework) part-time, up to two days per week after six months of employment. In order to telework, you must have a securely configured high-speed internet connection and work from an approved location inside Pennsylvania. If you are unable to telework, you will have the option to report to the headquarters office in Pittsburgh.
+ Salary: In some cases, the starting salary may be non-negotiable.

REQUIRED EXPERIENCE, TRAINING & ELIGIBILITY
QUALIFICATIONS
Minimum Experience and Training Requirements:
+ Six months as an Aging Care Manager 1 or a County Caseworker 1; or
+ Successful completion of the County Social Casework Intern program; or
+ Six months of professional human services or nursing experience; and a bachelor’s degree in social sciences, behavioral sciences, human services, or a closely related field; or
+ An equivalent combination of experience and training.
