    CO State Veterans Community Living Center at Rifle

     

    The Colorado State Veterans Community Living Center at Rifle is located on Colorado's western slope in the scenic Colorado River valley. Rifle offers a mild climate, with plenty of clean mountain air and beautiful blue skies. Amidst this setting, the State Veterans Community Living Center at Rifle provides 89 beds of skilled nursing care, including an 18-bed Special Care Unit for residents with Alzheimer's Disease or related conditions. Serving veterans, their spouses, and eligible "Gold Star" parents, we are committed to caring for the whole person by addressing the personal and social needs of each resident, as well as their physical and medical requirements. All staff members of Rifle are committed to providing a life of quality and dignity for every resident.     Description of Job

    May be part-time and full-time positions available.

     

    The Client Care Aide II will provide direct restorative care and formalized therapy tasks as assigned. Performing tasks accurately, safely and in a timely manner. Specific care to ensure and perform passive and active range-of-motion, ambulation, special positioning techniques, splints, assistive feeding devices/adaptive equipment, ADL training, bowel and bladder management and training, restorative dining and restraint reduction.

    Additionally, as a Client Care Aide II, you can expect to:

    Perform basic assessments, including monitoring vital signs, taking blood pressure, measuring weight, intake and output, and basic skin assessments.

     

    + Perform basic nursing care: bathing, feeding, dressing, transferring, ambulating, positioning, lifting, range of motion, toileting, skin care, interacting socially to meet residents’ basic needs, and FMP Programs

    + Assist and mentor nursing staff in resident safety, importance of Park and Walk programs, resident transfer and mobility, assistive devices, mobility aids, equipment and gait belts. Perform CPR as needed.

    + Safety of residents and others by use of correct lifting techniques, monitoring the environment and resident whereabouts/activities, use of safety devices such as lifts, etc.

    + Perform basic resident assessments such as weights, bowel and bladder incontinence, intake and outputs, vital signs, and records the same. Records hall sheet data each shift.

    + Implement proper infection control practices via the use of standard precautions. Equipment checks, equipment maintenance, and placement of equipment, i.e. mattresses on beds, foot cradles, etc.

    + Communicates with others to ensure proper resident care and work completion. Inform RN’s and LPN’s of findings and changes in resident condition.

    + Maintain therapeutic milieu through appropriate interpersonal skill use, monitoring of environments and assisting residents to maintain rooms and possessions.

    + Maintain resident care and equipment such as cleaning oxygen concentrators, filling portable oxygen tank, maintain/disinfect tubs and showers, changing lift batteries, etc.

    + Participate in restorative meeting/GAPA meetings

    + Transports residents for medical appointments, etc. in state vehicles when the need arises.

    + Attend educational training and staff meetings to maintain current on techniques and policies. Provide adequate inter-shift communication

    + Other duties as assigned

     

    Minimum Qualifications, Substitutions, Conditions of Employment & Appeal Rights

    Special Qualifications

    + Current & active Certified Nurse Aide License (NA)- renewal required.

    Education/Licensure/Certification/Experience:

    + High school diploma or General Educational Diploma (GED)

    + One year of experience in an occupational field related to the work assigned to the position

    Substitutions:

    + Current, valid certification as a Nurse Aide (CNA) from the Colorado Department of Regulatory Agencies will substitute for the high school diploma or GED

    + Current, valid licensure as a Practical Nurse (LPN) from the Colorado Department of Regulatory Agencies or any state participating in the Nurse Licensure Compact will substitute for the experience requirement

    Preferred Qualifications

    + Knowledge and ability to appropriately carry out assigned resident care based on the residents' conditions; to use judgment and to accurately recognize, report and record relevant resident information.

    + Knowledge and ability to recognize urgent or emergent resident care situations, seek assistance of the RN and/or MD/DON, and initiate appropriate emergency interventions as directed.

    + Knowledge of quality standards, procedures, and protocols

    + Knowledge of specialized client care applicable to the work situation

    + Skill in initiating, performing and completing assigned duties in providing care to residents.

    + Ability to effectively work with an elderly patient population of varying physical, mental and emotional abilities and staff from multiple work units in the facility to achieve quality care planning and delivery

    + Ability to observe, identify and respond to the resident's needs for care, including medication, equipment-assisted care and patient/family education.

    + Superior verbal, listening and written communication skills with all health care providers, residents, families and general public.

    + Demonstrated ability to display professional conduct at all times when dealing with fellow staff members, patients, families and the general public.

    + Demonstrated ability to maintain confidentiality at all times.

    + Long-term care/nursing home experience is highly desirable

    + Effective problem solving skills

    This position requires the following asconditions of employment:

    + CDHS employees (all Direct contact with vulnerable persons):

     

    CBI name check and fingerprint check, ICON Colorado court database, Medicare fraud database, Reference Checks, Professional License verification (licensure requirements), Drug Screen, PREA (Division of Youth Services),Trails check (direct contact with children), CAPS (direct contact with adults – Mental Health Institutes, Regional Centers, Veterans Community Living Centers)

     

    + This position must maintain a current CPR Card and perform CPR to provide adequate compressions and ventilations per CPR Guidelines

    + Must pass TB test.

    + Shift Work: Day, evening, and night shifts to include holiday & weekends as needed

    + Current/Valid and unrestricted US Driver’s License

    + Travel: May have to transport residents in state vehicles.

    + This position is considered "essential service designation" and therefore, this position will be required to report without delay or interruption to provide essential or emergency services to ensure health, safety, and welfare.

    + On-call Status: May sign up for on-call

    + Former State employees who were disciplinarily terminated or resigned in lieu of termination must disclose the information on the application and provide an explanation why the prior termination or resignation should not disqualify the applicant from the current position. Absent extraordinary circumstances, prior disciplinary termination or resignation in lieu of termination will disqualify the applicant from future State employment with CDHS.
