Summary

Manages the overall on-boarding process for patients admitted onto hospice program.

+ Review/verify admission information- data

+ Managers team for designated territories, including, RN's LPN's Social Workers, Clerical Staff, Etc.

+ Ensure notification to clinician of pre-admission documentation needs

+ Ensure visit schedule is generated

+ Review referral information; identify resources necessary for patient admission

+ Ensure Communication of face to face encounters

+ Participates in orientation of new hospice staff.

+ Document in clinical record all interactions with patient/family/staff.

+ Serve as a liaison between Intake services and Clinical team supervisors.

+ Provide follow-up information to referrer (non-M.D.) and document same.

+ Provide patient updates to referrers

+ Coordinate assessment and documentation requirements for continued Medicare/Medicaid Recertification’s.

+ Prepare statistical reports for Corporate Compliance Regulations @ Home and QA & I.

+ Maintain overall supervision of discharge medical record completion prior to final filing.

+ Performs other related duties as assigned.

Requirements

Qualifications:

+ Current NYS RN License

+ Current Driver's License.

+ Baccalaureate Degree in Nursing from an accredited school is preferred.

+ A minimum of 7-10 years’ experience in nursing which includes minimum of 5 years of hospice

+ Management experience preferred

+ Working knowledge of hospice regulations and Conditions of Participation

+ Exhibits an ability and attitude relative to the spirit of caring and tradition at Calvary Hospital as required by specific job duties and functions.

Salary Range:

$127,817 - $154,317 annually
