    The role you'll contribute:

    The Patient Care Representative (PCR) will focus all of their work with the AdventHealth Medical Group practice providers and team to provide critical analytical support. They will serve our consumers by providing outreach to patients to reduce care gaps and assist with risk adjustment factor (RAF) and Hierarchical Condition Categories (HCC) AF/HCC proposals, medication adherence, and wellness visits.

     

    The PCR will collaborate with practices and the leadership teams on patient care issues, best practices, and workflow to ensure organizational goals and objectives are met at the practice level. The PCR displays a caring and courteous attitude and represents the medical group in a positive manner.

    The value you'll bring to the team:

    Conducts pre-visit chart review of patients including identification of all needed preventive health maintenance and immunizations.

     

    Completes daily chart preparation including the Physician Attestation Form (PAF) or Attestation prior to the patients appointment.

     

    Perform HEDIS medical record reviews, data collection, chart abstractions, reporting, and analysis.

     

    Coordinate and perform onsite medical record reviews and medical record collection at provider offices to support optimum MRA/HEDIS and quality metric reporting.

     

    Identify patients overdue for visits, labs, or referrals and arrange for follow-up services as appropriate to assist with care gap closure, delivery of quality care, and overall health needs of the patients.

    Qualifications

    The expertise and experiences you'll need to succeed** **:

    + High-school diploma or equivalent Healthcare environment experience or education

    + Billing and coding experience required

    + Insurance verification experience required

    + Knowledge of quality metrics

    + Knowledge of HCC

    + Knowledge of quality initiatives

    + Understanding of insurance requirements
