    Physician-Primary Care-Medical Evaluation Board (MEB) Physician

    SITE OF SERVICE:

    • Walter Reed National Military Medical Center, Bethesda, MD and DiLorenzo Pentagon Health Clinic (DPHC), Washington, DC.

    • HCWs shall receive notification two weeks prior to the reassignment to another location with a 40-mile commuting radius of their assigned MTF.

    POSITION QUALIFICATION/REQUIREMENTS:

    • **Degree/Education:** Doctor of Medicine (M.D.) or Doctor of Osteopathy (D.O.).

    o Graduate from an accredited medical school in the United States or Canada.

    o This degree must have been accredited by the Council on Medical Education of the American Medical Association; Association of American Medical Colleges; Liaison Committee on Medical Education; Commission on Osteopathic College Accreditation of the American Osteopathic Association, or an accrediting body recognized by the U.S. Department of Education and the Accreditation Council for Graduate Medical Education (ACGME) at the time the degree was obtained (e.g. Royal College of Physicians and Surgeons of Canada (RCPSC), College of Family Physicians of Canada (CFPC).

    o A Doctor of Medicine or equivalent degree from a foreign medical school must provide education and medical knowledge substantially equivalent to accredited schools in the United States. Evidence of equivalency to accredited schools in the United States is demonstrated by permanent certification by the Educational Commission for Foreign

    o Medical Graduates, a fifth pathway certificate for Americans who completed premedical education in the United States and graduate education in a foreign country, or successful completion of the U.S. Medical Licensing Examination.

    • **Internship/Residency:**

    o Successful completion of an internship and residency program (corresponding to the specialty required) which has been approved by the Accreditation Council for Graduate Medical Education or the Committee on Postdoctoral Training of the American Osteopathic Association.

    o Subsequent to obtaining a Doctor of Medicine or Doctor of Osteopathy degree, a candidate must have had at least 1 year of supervised experience providing direct service in a clinical setting (i.e., a 1-year internship or the first year of a residency program in a hospital or an institution accredited for such training).

    o For purposes of this requirement, graduate training programs include only those internship, residency, and fellowship programs that are approved by accrediting bodies recognized within the United States or Canada. Descriptions of such programs are described below.

    o An internship program involves broadly based clinical practice in which physicians acquire experience in treating a variety of medical problems under supervision (e.g., internal medicine, surgery, general practice, obstetricsgynecology, and pediatrics). Such programs are in hospitals or other institutions accredited for internship training an appropriate accrediting body.

    o A residency program involves training in a specialized field of medicine in a

     

    hospital or an institution accredited for training in the specialty by an appropriate

     

    accrediting body.

     

    o A fellowship program involves advanced training (beyond residency training) in a

     

    given medical specialty in either a clinical or research setting in a hospital or an

     

    institution accredited in the United States for such training.

     

    • **Board Certification:** Board eligible or board certified as required.

    • **Additional Certifications** : Must have the following hands-on certification through the American Heart Association or American Red Cross

    o Current Basic Life Support (BLS)

    o Current Advanced Cardiac Life Support (ACLS)

    • **Experience:** Minimum of 1 year experience within preceding three (3) years in the specialty. In addition, minimum of five (5) years’ experience, within the last seven (7) years, working in military medical or clinic facility. Shall have a strong working knowledge of the military healthcare system.

    • **Licensure:** Current, full, active, and unrestricted license to practice medicine

    • **U.S. Citizenship** **:** Shall be a U.S. citizen

    • **English Language Requirement:** Shall be able to read, write, speak and understand English well enough to effectively communicate with all patients and other health care providers.

    • **Physical Capability:** Shall be physically capable of standing and/or sitting for extended periods of time and physically capable of performing all services required under the contract.

    Additional Qualifications:

    • Have working knowledge of Medical Board process of the Navy, Army, Air Force, Coast Guard, Public Health Service (PHS), and National Oceanic Atmosphere Administration (NOAA).

    • Have a working knowledge of the International Classification of Diseases (ICD-9-CM Codes) to read the diagnoses code listed in the medical board report.

    • Knowledge of references and instructions which pertain to the Medical Boards Department, SECNAVINST 1850.4E, DOD INSTRUCTION 1332.38, MANMED CHAPTER 18, CHANGE 120, Correspondence Manual, Standard Operating Procedures, local WRMMC instructions and policies, Army Regulation 40-501, “STANDARDS OF MEDICAL FITNESS (*RAR 003, 08/04/2011)”, DODD 6130.3 and DODI 613. 4.

    • Working knowledge of requirements for processing a medical board report on U.S. Army,

    • U.S. Air Force, U.S. Coast Guard, and U.S. Public Health Service Officer, which differ from the processing of Navy and Marine Corps medical board reports.

    • Skill in interviewing, examination, assessment, and management of patients with general medical and surgical health problems. Seriously ill patients shall be managed in consultation with or by direct referral to specialty physicians.

    DUTIES:

    • The HCW shall perform the following procedures, as required, and in strict compliance with standing orders and protocols set forth by the MTF to include, but will not necessarily be limited to the following:

    o Work under the general professional direction of the Chief of Military Patient Personnel Division (Medical Boards) / Patient Administration Department.

    o Review diagnoses code listed in the medical board report, using ICD-9-CM codes daily as required. Ensure that the ICD-9 code is appropriate for the patient’s medical diagnosis.

    o Follow established guidelines/standards, relating to Army/Navy retention standards, in accordance with regulations, Army Physical Disability System, Disability Evaluation System, Veterans Administration Schedule for Rating Disabilities (VASRD) and other essential policies, for the purposes of conducting MEBs.

    o Compile reports, ensuring that a copy of the completed medical board report for each member is maintained on file, and that a copy of the medical board report cover sheet is properly coded and forwarded to the NNMC as required annually.

    o Review available medical records of all military personnel referred by medical staff for possible disability processing and assesses the condition of the service member in accordance with applicable regulations.

    o Make recommendation if the Soldier/Sailor meets retention standards, and if further evaluation or treatment is likely to result in meeting retention standards.

    o Make appropriate profile recommendation for disability processing.

    o Review the individual’s record on the Temporary Disability Retirement List (TDRL), who has been directed to undergo a periodic physical examination.

    o Review medical records and examines service members being referred for fitness for Duty (FFD) and Active-Duty Medical Extension (ADME) evaluations.

    o Make further treatment recommendations examinations, utilizing advice from specialty clinics, if further treatment is likely to result in meeting retention standards or complete necessary MEB processing.

    o Act as a consultant to Director of Patient Affairs and staff or referring physicians concerning medical retention or fitness for duty issues, as necessary.

    o Become the subject matter expert, who become frequently called upon to assist in MEB cases of significant public interest, high visibility or complexity.

    o Assume responsibility as the Primary Care Manager (PCM) for service members undergoing MEBs and their families.

    o When requested, by the patient or healthcare provider, meet with patient’s families or Command representatives to coordinate, explain and answer questions regarding the administrative aspects of military healthcare.

    o Provide advice and opinions upon request by the Director of Patient Affairs or the referring physician in support of patient care, treatment, or fitness for duty.

    o The HCW shall be responsible for providing contracted services during the required work hours, including as designated by the medical treatment facility (MTF), and shall provide immediate evaluation and management of emergent problems as they occur.

    o Shall assume additional administrative or clinical duties as requested or needed to ensure sustainment of normal clinic operations and ensure compliance with JC standards, to include, but not limited to, managing daily schedules, reviewing and disposition consults, monitoring appointment utilization and patient access to care, conducting mandatory clinical training, monitoring clinic supplies and equipment.

    CORE DUTIES:

    • Perform a full range of physician services in accordance with privileges granted by the MTF.

    • Direct, perform, or assist in the instruction of other health care professionals within the scope of the clinical privileges or responsibilities.

    • Examine, diagnose, treat or prescribe courses of treatment within the scope of training, experience, and privileges Provide preventive and health maintenance care, including annual physicals, positive health behaviors, and self-care skills through education and counseling.

    • Order diagnostic tests as applicable as well as request consultation or referral with appropriate physicians, clinics, or other health resources as indicated.

    • Technically proficient in directing and teaching other medical staff, providing educational lectures, and participating in the provision of in-service training to clinic staff members. Such direction and interaction will adhere to Government and professional clinical standards and accepted clinical protocols.

    HOURS:

    • Walter Reed National Military Medical Center (WRNMMC) and DiLorenzo Pentagon Health Clinic (DPHC): Operates 24 hours daily/7 days weekly/365 yearly/366 days in leap years for inpatient services including holidays.

    • Full-time contractor personnel typically work 40 hours per week unless mission dictates changes.

    • Operates Monday – Friday 7 a.m. to 4 p.m., closed on weekends and federal holidays. Duty hours include a one-hour, unpaid lunch period, excluding federal recognized holidays and other times the clinic is authorized to be closed by the Clinic Director.

    • Duty hours will not exceed 80 hours in a two-week period. Hours worked do not include travel to and from the work location.

    • Due to the nature of patient care, HCWs may be required to work beyond 40 hours per week; however, hours will not exceed 80 hours in a two week period.

    • Hours exceeding 40 hours per week will be coordinated and adjusted with the Contractor, Government supervisor, and COR to not exceed 80 hours within the current two-week period.

    • HCWs will be paid only for the hours worked at the established rate for the CLINs. Overtime is not authorized.

    SPECIAL REQUIREMENTS/SKILLS

    Must be comfortable in a fast-paced, dynamic environment. Must be able and willing to reprioritize on short notice and work on multiple simultaneous projects. Flexible and able to work with various personalities. Teamwork skills required. Time management skills required. The ability to meet deadlines in a deadline intensive environment is essential. High level of adaptability and willingness to embrace change in a fast-paced, demanding environment.
