    Responsibilities:

    + Support initiatives of SAH Mission Integration Department in collaboration with Catholic Charities in facilitating the daily operation of the Pinellas Hope Community and The Medical Respite.

    + Responsible for the oversight and assessment of client medical needs and preventative education; such as, diabetes management, heart failure, advance directives, for the Pinellas Hope community.

    + Utilizes BayCare Anywhere for assessment when indicated and collaborate with Pinellas Hope staff to meet the needs of clients and working with multidisciplinary community resources.

    + Implements evidence-based care of The Medical Respite Population relative to medical intake procedures and assists with organizing all aspects of patient care services. Develops an individual Plan of Care that assists clients in establishing short and long-term goals and supports them with remaining on the path to achieve goals.

    + Maintain performance measures on patient outcomes to help gauge program performance.

    Minimum Qualifications:

    + Required Experience includes 3 years Clinical RN experience with adult population.

    + Preferred experience includes 1 years Public Health or Population Focus.

    + Required - Associates - Nursing; Or - Diploma - Nursing; Preferred - Bachelors - Nursing
