Title: Home Health RN, Farmington Office (Anson/Madison coverage)

What You’ll Do:

+ Provide skilled nursing services to the patient in their own home under the direction of a physician’s plan of care.

+ Develop plans of care with realistic/measurable outcomes in collaboration with the patient/family.

+ Provide education and promote patient’s rights and responsibilities for informed decision making in the plan of care.

+ Assess and identify the patient’s holistic health care needs.

+ Demonstrate competency in assessment and technical skills.

+ Possess and utilize knowledge of pain control & symptom management.

What You’ll Bring:

+ Current R.N. license in the State of Maine.

+ ANA certification in Home Health or Community Health Nursing encouraged.

+ Current CPR card required.

+ One year of recent clinical nursing experience required.

+ A reliable car with insurance.
