Responsibilities

Direct community health policy, community-based health initiatives and health programs aimed at promoting health and preventing disease.

+ Implement strategic initiatives to address needs identified in Community Health Needs Assessment.

+ Develop and implement programming to increase access to transitional care and community-based interventions for high-need patients.

+ Develop and implement programming to support integration of behavioral health services into transitional care programs.

+ Identify funding sources, develop and submit proposals to external funders to enhance community-based programs and resources.

Requirements

Education:

+ High School diploma equivalency with 5 years of applicable cumulative job specific experience required, with 2 of those years being in leadership/management OR Associate's degree/Bachelor's degree with 3 years of applicable cumulative job specific experience required, with 2 of those years being in leadership/management.

Additional Preferences

+ Masters degree in health care related field strongly preferred

+ Experience in health care operations

+ Experience in community health, health equity and program development

+ Experience with medical access programs, health equity grants

+ Strong project management skills

+ People management experience
