    Job Description

    The **UW Medical Center-Montlake Pharmacy Department** is seeking a highly-motivated **Clinical Pharmacist - Ambulatory Oncology.**

    WORK SCHEDULE

    • Full-Time / 40 Hours Per Week

    • Day Shift

    POSITION HIGHLIGHTS

    + Provide pharmaceutical care in our ambulatory gynecology-oncology, neuro-oncology and genitourinary oncology patient populations

    + Be a vital part of the outpatient multidisciplinary patient care team

    + In addition to direct patient care responsibilities including oral chemotherapy management, our pharmacists provide drug therapy consults, participate in treatment protocol development, provide patient/family/staff education, facilitate transitions of care, and precept pharmacy students and residents

    PRIMARY JOB RESPONSIBILITIES

    + Ensure appropriate drug therapy and nutrition, including indication, dose, drug interactions, and drug allergies.

    + Evaluate pharmacotherapeutic regimens for effectiveness and adverse drug events, and intervene as needed.

    + Utilize the medical record and other appropriate documentation systems to communicate with healthcare professionals and to document specific pharmacotherapeutic recommendations.

    + Provide patient/family counseling and education

    + Update patient medication lists and reconcile medication orders upon hospital admission and discharge

    + Provide primary preceptorship of pharmacy residents and student pharmacists.

    + Perform duties of a Pharmacist 2.

     

    Strong candidates will have completed an ASHP-accredited PGY2 residency in oncology pharmacy practice.

    REQUIRED POSITION QUALIFICATIONS

    Two years of experience in pharmacy practice. Months spent in the completion of an American Society of Health System Pharmacists accredited residency training program may substitute month-for-month for the experience.

    LEGAL REQUIREMENT

    State of Washington Pharmacist License.

    Compensation, Benefits and Position Details

    Pay Range Minimum:

    $151,716.00 annual

    Pay Range Maximum:

    $217,092.00 annual

    Other Compensation:

    Night Shift Premium - $8.00/hour; Evening Shift Premium - $1.75/hour; Weekend Shift Premium - $1.50/hour
