    Description:

    Position Purpose

    Mercy Care Atlanta is seeking a compassionate and dedicated **Certified Peer Outreach Specialist Case Manager** to join our outreach team. This role is designed for individuals with lived experience in mental health and/or substance use recovery who are passionate about supporting persons experiencing homelessness (PEH). Through street outreach and peer-based engagement, you will help individuals navigate recovery, access essential services, and build self-advocacy skills.

    What You Will Do

    + Conduct outreach in encampments, under bridges, and on sidewalks to engage individuals facing mental health and/or substance use challenges.

    + Use your lived experience to provide emotional support, motivation, and recovery coaching.

    + Assist clients in navigating healthcare, behavioral health, and social service systems.

    + Facilitate connections to treatment programs, recovery services, and peer support groups.

    + Maintain accurate documentation of interactions and progress while upholding confidentiality.

    + Collaborate with outreach teams and service providers to coordinate care and advocate for client needs.

    + Manage an active caseload of approximately 20–25 clients, providing consistent follow-up and support.

    Minimum Qualifications

    + High School Diploma or equivalent.

    + Certified Peer Specialist (CPS) certification from the State of Georgia.

    + At least 3 years of experience in mental health or social services.

    + Strong communication, problem-solving, and interpersonal skills.

    + Compassionate, non-judgmental, and culturally sensitive approach.

    + Valid Georgia driver’s license; ability to operate company vehicles, including a 12-passenger van.
