• About the Role

We’re looking for a dedicated Nurse Navigator to join our team and help us continue delivering the level of care our patients and families deserve. In this role, you’ll assist patients with a diagnosis of breast cancer. The Nurse Navigator provides support and education to patients and their families from diagnosis through survivorship and continues to be a resource after completion of treatment.

• What You’ll Do

+ Integrates patient-centered care in the nursing processes to include the patient(s) and family in care decision, incorporating evidence-based practices to achieve safe and effective patient and process outcomes.

+ Advocates for the unique needs of individuals and population served: anticipates and removes barriers and act to assure service(s) access and service effectiveness.

+ Communicate with physicians about patients' potential for referral/follow-up treatment, and schedules follow-up appointments.

+ Provides referrals/support for follow-up care for patients who are uninsured or underinsured.

+ Functions as liaison for management staff, administration, physicians, and community organization.

+ Coordinates efforts for patient care plan and promotes quality improvement and educational efforts.

+ Compiles data, tracks outcomes, and makes recommendations for process improvement.

•œ… What You Bring

+ Graduate of an accredited school of nursing.

+ Current NYS Registered Nurse (RN) license.

+ Bachelor's Degree in Nursing Preferred.

+ Must have three (3) or more years of nursing experience. Outpatient oncology experience required to include Radiation, Oncology, or Surgical Oncology.

+ Participates in local (in addition to breast cancer conference attendance), state, regional, or national breast-specific educational programs annually. OCN or CBCN preferred, required within one (1) year of hire.
