    Penn Presbyterian Medical Center (PPMC) has an outstanding opportunity for a PRN Social Worker MSW to join the Case Management Social Work team and work alongside some of the best medical providers in Philadelphia.

    Summary:

    Responsible for discharge planning and contributing to the patient's treatment plan through and understanding of social, psychological, and/or environmental needs related to the impact of illness and need for hospitalization. Professional activities will be carried out in accordance with the code of ethics established by the National Association of Social Workers.

     

    Our Per Diem need is Weekday availability from our staff.

    Responsibilities:

    + Coordinates the discharge planning process to ensure a safe and timely transition from hospital to home, community, or institutional setting.Collaborates and cooperates with other members of the interdisciplinary health care team to ensure continuity of care.Identifies and attends to the psychological needs of patients and families.Provides counseling to patients, families, and groups.Responsible for documentation in the medical record, statistics and reports as required.Facilitates legal matters, financial entitlements and complies with grants and contracts when applicable.Maintains relationships with community agencies and keeps up to date on resources.Uses effective interpersonal skills in all interactionsEnhances own professional development and maintains formal and informal continuing education for self-improvement.Performs duties in accordance with Penn Medicine and entity values, policies, and proceduresOther duties as assigned to support the unit, department, entity, and health system organization

    Minimum Requirements:

    + MSW – Master’s Degree in Social Work required

    + Health Care experience in field instruction and/or employment in a health care setting preferred

    + LSW preferred

    Essential Skills and Abilities

    + Excellent communication and writing skills, ability to operate with limited direction; ability to multi-task; high level attention to detail

    + Extensive knowledge of medical terminology, human anatomy and physiology, and clinical disease processes

    + Knowledge and understanding of human development and behavior, and ability to make psychosocial assessments
