    Salary Range: $88,000.00 - $102,800.00

    Position Summary

    Evaluates and treats cardiopulmonary (lungs and heart) health. This includes teaching patients and their families about treatment and maintenance of pulmonary health problems, including the use of respiratory therapy equipment and medications.

     

    This role coordinates inpatient care in an effort to reduce length of stay, reduce re-admissions and Emergency Department visits, and improve the quality of life for patients with chronic disease.

    Job Responsibilities

    + Confirms patient diagnosis and works with management team to ensure standards of care are met and that there is a rapid and appropriate transition from intravenous to oral antibiotic and corticosteroid therapy.

    + Discusses disease specifics and self-management with patient. Educates patient/families on the proper use of nebulizers and inhalers and assessing home equipment/oxygen needs. Reviews medication regimens and evaluates activities of daily living.

    + Assists in decisions on post-discharge care on whether the patient should go home, go to subacute rehab or a nursing home. Arranges for outpatient pulmonary rehabilitation.

    + Ensures timely and complete discharge and scheduling of follow up appointments. Works closely with case management and social work teams to meet the needs of the patient before and after discharge.

    + Selects the most appropriate oxygen delivery equipment to meet patient needs. Makes routine phone calls and home visits to assess patient health and their home environment.

    + Serves as point of contact for referring physicians, patients and caregivers to provide assistance with accessing clinical and supportive care services for patients and availability of community resources.

    + Collaborates with multi-disciplinary teams to develop and maintain evidence-based clinical pathways and educational materials, and promote the use of pathways with physicians.

    + Serves as a resource for clinically-based questions pertinent to area of practice or determines appropriate resources.

    + Creates/maintains a registry to identify and manage patients in the disease management program to assess processes and outcomes, and identify opportunities for improvement.

    Education

    + Associate's Degree

    Experience

    + Minimum of associate's degree from a respiratory therapy program registered by the Board of Regents and/or accredited by the national respiratory therapy professional association.

    + Bachelor's degree preferred.

    + Minimum five years working in a clinical setting.

    + Experience with multi-disciplinary teams or leadership experience preferred.

    Knowledge, Skills and Abilities

    + Ability to work effectively within a multidisciplinary team.

    + Excellent communication skills (both verbal and written).

    Licenses and Certifications

    + Current NYS license and registration certificate as a Respiratory Therapist (RT).
