    Job Summary and Qualifications

     

    The Pharmacy Clinical Manager is responsible for designing, implementing and managing the pharmacy clinical programs. This includes formulary management, policy and procedure development, pharmacist clinical practice development, and education across the healthcare spectrum. The Clinical Manager shall also assist in managing the pharmacy when the Pharmacy Director or designee is unavailable.

     

    + You will interpret orders and transcribe or verify them accurately and efficiently into the pharmacy computer system You will accurately compound and dispense pharmaceuticals

    + You will comply with facility policy and procedures and regulatory requirements You will monitor drug therapy regimens for contraindications, drug-drug interactions, drug-food interactions, allergies and appropriateness of drug and dose

    + You will Provide clinical consultation and clarification to practitioners. Suggests appropriate, cost-effective therapeutic alternatives to medical staff as needed.

    + You will develop, guide, and support pharmacists in providing clinical consultation and clarification to practitioners. Suggests appropriate, cost-effective therapeutic alternatives to medical staff as needed.

    + You will sustain the hospital formulary, minimizing nonformulary procurements, utilizing therapeutic interchanges and promote rational drug therapy selection.

    + You will support participation in daily multidisciplinary and bedside rounds (may need to attend).

    + You will identify, design and analyze drug utilization reviews to promote rational drug therapy and determine action plans based on results.

    + You will provide accurate and timely medication information to healthcare staff and patients.

    + You will develop, manage and mentor pharmacy clinical programs such as drug consults, nutrition support, pharmacokinetics, anticoagulant monitoring, pain management, IV to PO, renal dosing and other programs as determined appropriate based on patient population and literature guidelines.

    + You will develop, manage and mentor antimicrobial management program (AMP)

    + You will support and participate in the goals of the organization’s quality improvement, risk management and patient safety program including but not limited to National Patient Safety Goals, Core Measures and HCAHPS medication-related initiatives

    + You will be responsible for design, analysis and reporting of adverse drug reactions, medication errors and pharmacy quality initiatives. Mentors others in their roles for these programs

    + You will develop and actively participate in training and mentorship programs for pharmacists, pharmacy students, residents, technicians, nurses and physicians Responsible for assessment of participants in these programs

    + You will Develop and document staff competency on an ongoing basis and develop improvement plans when deficiencies in competencies are identified

    + You will be the primary preceptor for pharmacy students and/or residents as it relates to clinical pharmacy services. Responsible for designing and managing program and mentoring student/resident as well as other preceptors.

    + You will maintain professional competency sufficient to meet current and future needs of the facility.

    + You will develop and manage specialty pharmacy practice programs as related to clinical pharmacy services.

    + You will provide basic technical support for automated dispensing machines and other pharmacy computer systems

    + You will effectively and proactively intervene in conflicts between pharmacy and medical staff regarding drug therapy issue resolutions.

    + You will comply with all regulatory standards.

    + You will work with the Pharmacy Director and Operations Manager to identify and develop processes to meet or exceed all regulatory standards.

    + You will participate in quality and regulatory reviews and audits

    + You will prioritize work and lead others to comply with facility medication turn-around time policy and procedure.

    + You will identify areas for improved efficiency

    + You will supervise and assist pharmacy technicians and other support staff effectively

    + You will collaborate with pharmacy leadership to establish pharmacy goals and objectives and responsible for implementation and monitoring.

    + You will participate, chair, consult as appropriate with various nursing and medical staff committees as necessary.

    + You will be responsible for development and review of physician order sets and other evidenced based medication use guidelines.

    + You will be responsible for routine publication of the pharmacy newsletter and/or other forms of written and electronic communication.

    + You will be responsible for drug class reviews and drug monographs

    + You will be responsible for managing and monitoring the drug formulary.

    + You will be responsible for the development, implementation, and monitoring of pharmacy clinical initiatives and collaboration on pharmacy operations with Pharmacy Operations Manager as appropriate.

    + You will be responsible for the development and review of pharmacy policies and procedures

    What qualifications you will need:

    + 5+ years of experience

    + Completion of hospital PGY2/Pharmacy Specialty residency, or fellowship required

    + Board Certification required

    + Prior hospital pharmacy management preferred

    + PharmD

    + Current Texas Pharmacist License – required upon hire

    + ACPE IV certification required upon hire
