    The Critical Time Transition Program (CTTP) Director at the House of the Good Shepherd provides strategic leadership and clinical oversight to a multidisciplinary team supporting children and youth transitioning from hospital settings to community-based care. This role is grounded in trauma-informed, strengths-based, and relationship-centered practices, with a strong emphasis on family engagement, cultural responsiveness, and skill-building.

    Principal Assigned Responsibilities

    • Design, implement, and continuously improve the CTTP in alignment with New York State Office of Mental Health (OMH) regulations and agency policies.

    • Collaborate with statewide CTTP programs to inform and influence program evolution and best practices.

    • Ensure the coordination of incident investigations relative to allegations of abuse of inappropriate staff actions and other reportable incidents requiring special needs in accordance with agency policy.

    • Develop and maintain positive professional relationships with all community stakeholders, facilitating regular communication with stakeholders and advising feedback to improvement process/services.

    • Oversee the integration and performance of Critical Time Intervention (CTI) and Transitional Residential Setting (TRS) teams.

    • Provide clinical direction and supervision to ensure high-quality, evidence-informed care.

    • Ensure compliance with all federal, state, and local regulations, including OMH standards.

    • Participate in on-call duties as necessary.

    • Monitor and manage referrals, admissions, discharges, and transitions to ensure continuity of care.

    • Participate in the development of program budgets and ensure compliance relative to income and expenses through the course for the fiscal year within areas of responsibility.

    • Recruit, train, and support a diverse, high-performing team, prioritizing candidates with lived experience and cultural alignment with the populations served.

    • Foster a collaborative, inclusive, and mission-driven team culture.

    • Develop and manage complex program budgets.

    • Coordinate funding streams and billing processes across multiple payers.

    • Represent the program in agency-wide initiatives and external stakeholder meetings.

    • Actively support staff in maintaining a Sanctuary environment that demonstrates the Sanctuary Commitments and is grounded in the philosophy of Trauma Informed Care.

    • Other related activities assigned by the Vice President of Residential Services

     

    Relationships – Supervision and Coordination

     

    The Critical Time Transition Program (CTTP) Director reports directly to the Vice President of Residential Services and has regularly scheduled supervision with that person. The Director must maintain positive relationships with all treatment team members and will meet regularly with all directly reporting clinicians, supervisors, and employees. The Director must maintain positive working relationships with youth and families in treatment. He/she must be able to develop and maintain positive working relationships with service providers in the communities in which children return to live after discharge.

    Education and Experience Requirements

    • Master's degree in social work, Psychology, Counseling, or a related field (required).

    • New York State Clinical License (required).

    • Minimum of 3 years of post-Master’s clinical experience (preferred).

    • Minimum of 3 years of leadership or program management experience (preferred).

    • Demonstrated experience in care coordination and navigating hospital and community behavioral health systems (preferred).

    • Strong knowledge of trauma-informed care, family systems, and culturally responsive practices (preferred).

    Required Knowledge, Skills, Abilities and Competencies

    1. Commitment to the following and embracing the Seven Commitments of the Sanctuary Model.

    2. Strong clinical observational skills and the ability to assess functions of behavior and antecedents in order to implement preventative behavior intervention strategies.

    3. Working knowledge of generally accepted ethics and standards of social work practice.

    4. Demonstrated knowledge of developmental issues and proficiency in the use of assessment procedures and effective clinical interventions with children and their families.

    5. Ability to engage in collaborative working relationships with agency staff and service providers in children's home communities.

    6. Ability to work independently and confidently in various settings.

    7. Understanding of basic needs and developmental issues of children in a way that respects individuality.

    8. Asks age and developmentally appropriate questions and actively listens to children's responses.

    9. Develops a warm, positive, nurturing relationship with each child.

    10. Follows mandated reporting requirements regarding child abuse and neglect.

    11. Practices infection control, including appropriate hand washing procedures.

    12. Supports staff in applying knowledge of child development and learning in daily practice.

    13. Demonstrates knowledge of behavioral treatment, social skills, and age-appropriate developmental milestones of youth.

    14. Demonstrates the ability to form effective relationships with youth and families.

    15. Demonstrated knowledge of documentation requirements of licensing and accrediting bodies.

    16. Strong written, verbal, and organizational skills.

    17. Valid New York State driver's license.
