Summary of the work unit:
Located within the Colorado Department of Human Services, the Office of Civil and Forensic Mental Health (OCFMH) is responsible for policy development, service provision and coordination, program monitoring and evaluation, and administrative oversight for the public behavioral health system. OCFMH funds, supports and monitors numerous mental health and substance abuse community programs and providers. OCFMH operates the Colorado Mental Health Hospitals at Fort Logan and Pueblo. Mental Health Transitional living Homes , and the Forensic Services Division. OCFMH executes the States’ federal responsibilities as the State Mental Health Authority and State Substance Abuse Authority for the purposes of administering federal mental health and substance abuse block grant funds.
The Forensic Services Division is responsible for six departments including Forensic Community Based Services (FCBS), Jail Based Evaluation and Restoration (JBR), Outpatient Restoration (OPR), the Forensic Support Team (FST), the Court Services Department, and the Operations Department. The Court Services Department employs forensically practicing psychologists to provide evaluative services statewide, with staff in Pueblo, Denver, and other locations. Evaluators complete all court ordered forensic evaluations for defendants in criminal cases as so ordered by the court. These evaluations are performed in the jail, in the community, at CMHHIP, or at CMHHIFL and are performed by psychologists and psychiatrists, as mandated in the Colorado Statute, 16-8.5.
Our teams work within various settings including community agencies, jails, courts, and hospitals. The locations can include locked units and secure facilities and our clients may at times be unpredictable due to their mental health needs. Our team is required to make critical decisions in dynamic and fast-paced environments in order to ensure a high level of accountability for patient care. Our staff are required to maintain updated skills, knowledge, abilities, and competencies necessary to the position through continuing education, clinical supervision, literature review, and discipline and team development. This position may be required to travel throughout Colorado to meet the needs of the department.
Description of Job
Summary of the position:
This position is the centralized specialist responsible for all applicable financial and medical benefits (e.g., Medicaid, Medicare, Social Security Insurance (SSI), Social Security and Disability Insurance (SSDI), Veterans Administration (VA), etc.) are applied for, secured, and maintained for all FCBS clients in need of such resources. This position serves as the liaison between CMHHIFL, CMHHIP, staff, and community stakeholders regarding benefit acquisition and retention needs of our clients. This position requires specialized knowledge regarding the benefits application process and system in order to independently secure benefits in a timely manner, which includes acquisition forms, submissions processes, and subsequent collaboration with our medical records department, and ability to navigate an electronic health record (EHR).
This position engages in problem solving and critical thinking to ensure submissions are completed. Patients in need of such resources will be identified and this position communicates the status of such applications on a regular basis. It is expected this position develops working relationships across treatment teams and with various community stakeholders. This position works closely with treatment teams, social workers, patients, community benefits agencies, and requires skilled interpersonal communication. This position is responsible for educating and collaborating regarding benefits and proactively identifying and resolving potential barriers.
This position requires case coordination skills, identification and development of process and procedure, applying critical thinking and analytical skills, and communicating effectively with others. This includes determining how to create and coordinate tracking processes to ensure timely approval of patient benefits and ongoing cross departmental training. Lastly, this position analyzes problematic cases by anticipating barriers, assessing each barrier, and reaching conclusions that may result in new internal processes, systems, or education. The position operates independently in performing the full range of professional tasks as related to procuring patient benefits and interfacing with hospital staff, benefit agencies, residential facilities, and other community based resources.
Job Duties:
Client Benefit/Care Coordination:
+ Responsible for client in-reach with our hospitals when individuals adjudicated not guilty by reason of insanity have been identified as eligible and are preparing for release. In-reach includes information gathering, needs assessment, meeting attendance, and coordination of all required documentation associated with benefits acquisition to support client’s successful community integration
+ Serves as a subject matter expert on behalf of the FCBS team as it pertains to community-based benefits and financial assistance needs, as well as a liaison for our case management staff
+ Receives and tracks all referrals to FCBS from the inpatient teams
+ Oversees the maintenance of financial needs and benefits for those living in the community via stakeholder engagement, attendance in treatment team meetings, and avoidance of any lapses. In order to do so effectively, this position must also be familiar with stakeholder policies and processes as it pertains to patient funds and use of money
Benefits Submission and Tracking
+ Follows up on all submitted paperwork in a timely fashion, reviews documents for potential errors, and delivers and tracks applications through completion
+ Creates and utilize a tracking database system to ensure accurate and timely information is stored and communicated in a Health Insurance Portability and Accountability Act (HIPAA) compliant manner
+ Identifies, tracks, and compiles any additional requests received related to benefits application and acquisition (i.e., submitting additional documents, scheduling phone call assessments, regularly scheduled follow-up calls with various agencies, etc.). Tracking shall include receipt of referral, identification of benefit needs, completion and submission of all required documentation, final determination for each submission, and maintenance of any ongoing paperwork requirements to maintain all benefits. etc
+ Proactively identify barriers, formulate solutions, and effectively implement those solutions. Use of this database and ongoing conversations with external stakeholders will inform potential barriers, identify changes to the system, and this position will then develop plans to address such issues and improve processes
+ Responsible for knowing all legislative changes and attending any necessary training to effectively do the job
Budget, Special Programs, and Agency Liaison:
+ Responsible for the oversight of patient stipends, Rocky Mountain Human Services (RMHS) Momentum funding, and any other supplemental funding programs the department is able to access. This includes but is not limited to any contracted service providers and associated invoicing
+ Ensures there are strong tracking mechanisms in place to monitor all spending on behalf of the department and timely payment of all services
+ Develops tracking mechanisms to monitor and assess use of funds, identify trends, identify requests, and ensure the fidelity of the use of such program funds
+ Serves as the primary lead in developing/informing policy and processes associated with any FCBS related oversight of patient funds. This may include laws associated with representative payees, accessing patient accounts, tracking and monitoring any ledgers/use of funds, etc
+ Serves as the primary liaison for all benefits related agencies and may be required to hold regular meetings, staffings, and/or training with such agencies
Supervision and Training
+ Responsible for the supervision of IIB 03857, which shall include ensuring all established practices and policies are adhered to in full and the individual maintains a strong working knowledge to remain successful in the position. This includes administrative supervision, professional development, performance evaluations and plans, corrective and disciplinary actions, monitoring staff assignments and quota, position descriptions, recruitment, retention, interviewing, onboarding, training, and ensuring division-wide required training has been completed
+ Responsible for department specific audits and assists in the establishment and adherence to quality assurance standards, processes, and documentation. The incumbent must communicate goals, objectives, and division/department announcements through the most effective means (e.g., staff meetings, email, individual interactions, etc.), review productivity numbers, set expectations, and administer performance improvement plans as needed
+ Develops and conduct training sessions for both internal and external stakeholders pertaining to FCBS, benefits, and patient fund related information.
Client Engagement:
+ Leads client skill building groups/sessions in order to assist with enhancing their use of finances, budgeting, understanding their benefits, budget development, and money management
Other Duties as Assigned:

Preferred Qualifications:
