Responsibilities

The incumbent provides care treatment and service to Home Health and Hospice patients and their families in their place of residence within their scope of practice. Functions as a member of the interdisciplinary team.

Qualifications

Experience:

+ Two (2) years LVN Experience within the last 5 years

+ Recent experience as an LVN in a Home Health or Hospice Agency preferred.

Licensure:

+ Currently licensed as a LVN in California

+ Current California drivers license possession of automobile insurance & access to an operating motor vehicle.

+ Current CPR/BLS card approved through the American Heart Association.

Pay Range

$43.12 - $48.64 /hour
