    Overview

    Purpose Statement/Position Summary:

    The Program Manager oversees the patient and family education program, with responsibility for planning, implementation, and evaluation of the patient/family education courses for patients across the care continuum Leads regular needs assessments to understand changes in patient populations and associated family learning needs and quality/safety trends for home-based care. Serves as a consultant to inpatient, ambulatory, and community settings to ensure continuity of care and programmatic offerings that minimize harm in the home setting and optimize patient outcomes and the patient/family experience. Leads continuous quality improvement initiatives to ensure that educational content is accessible, relevant, and evidence-based and uses a multimodal teaching approach to meet learning needs of a diverse population of patients and families. Tracks, analyzes, and reports program metrics to demonstrate impact and program sustainability.

    Minimum Qualifications/Work Experience:

    • Required:

    • 5+ years’ experience in nursing, with at least 3 years’ experience in pediatrics.

    • Strong organizational, data analysis, and project management skills

    • Excellent verbal and written communication skills

    • Preferred: Experience with patient/family education

    Education/Licensure/Certification:

    • Required:

    • RN prepared at the graduate level in nursing or a related field AND certified in area of specialty

    • California RN Licensure

    • BLS certification

     

    Pay Scale Information

     

    USD $104,395.00 - USD $178,963.00

     

    CHLA values the contribution each Team Member brings to our organization. Final determination of a successful candidate’s starting pay will vary based on a number of factors, including, but not limited to education and experience within the job or the industry. The pay scale listed for this position is generally for candidates that meet the specified qualifications and requirements listed on this specific job description. Additional pay may be determined for those candidates that exceed these specified qualifications and requirements.
