    Description:

    Position Purpose

    The Clinical Coordinator is accountable for coordinating all aspects of stroke and sepsis care. This position will lead the collaboration between nursing, medical staff, and other disciplines throughout the organization to optimize performance throughout the continuum of care. Responsible for maintaining Joint Commission certification as a Primary Stroke Center and maintaining evidence-based performance expectations for the hospital’s Sepsis Program. This position will also support and serve as a resource for other Joint Commission certifications within the organization.

    What you will do

    + Develop and evaluate clinical standards, care guidelines, policies and procedures using evidence-based information

    + Participate in the identification, investigation and resolution of adverse occurrences for stroke or sepsis patients

    + Lead continuous performance improvement for the Stroke and Sepsis Program through concurrent record reviews, data collection and analysis to ensure key performance measures are met

    + Develop and provide education to the community about stroke prevention and recognition

    + Provide stroke risk screening programs to the public

    + Coordinates regional stroke outreach activities

    + Provide required data and documentation to accrediting bodies upon request

    + Work collaboratively with patients and their families, health care providers, and community groups to facilitate stroke and sepsis care and prevention

    + Serves as quality representative and key content expert for other Joint Commission certification programs within the organization

    Minimum Qualifications

    + Connecticut RN license required, Bachelor’s Degree in Nursing required

    + At least five years hospital clinical experience in Critical Care or Emergency Services experience preferred

    + Clinical experience with stroke and sepsis patients preferred

    + Experience in data collection and analysis

    + Experience in teaching and/or clinical program development preferred
