    If you're looking for a way to make a positive change in your community, LSI is looking for you. Join our Early Childhood team as a Care Coordination Liasion! This position will serve Muscatine, Scott, Cedar, Clinton and Jackson counties.

    What You'll Do

     

    The Care Coordination Liaison will provide outreach to the community with a special focus on child care and preschool settings for children ages 0-5 experiencing a behavioral and/or mental health crisis. The Early Childhood Liaison will use assessment and planning skills to determine the family risks, provide referrals and create care coordination plans to help each family. After receiving a referral, the Early Childhood Liaison will provide extra short-term support while other services are identified for the family. This position will work with families in person, virtually and via phone. The Early Childhood Liaison will provide a comprehensive understanding of resources in the community, and be the bridge in maintaining a relationship between community resources, the program, and the families served.

     

    What You'll Need

     

    Preferred: Bachelor's degree in health or human service related field with a minimum of two years of related experience.
