    Responsibilities

    + Assist patients with physical therapy exercises and mobility techniques to improve their independence.

    + Communicate effectively with patients and team members to ensure clear understanding of care plans.

    + Maintain accurate patient records and ensure compliance with HIPAA and state regulations.

    + Adapt care plans to meet the unique needs of each patient's home environment.

    + Utilize EMR systems and mobile health apps for scheduling and reporting.

    + Provide empathetic and culturally sensitive care to diverse patient populations.

    + Ensure safe patient transfers and provide mobility support as needed.

    Essential Skills

    + Associate's degree in Physical Therapy Assisting or related field.

    + Physical Therapy Assistant Certification and PTA License.

    + Strong knowledge of patient care techniques and medical terminology.

    + Excellent communication skills for effective patient and team interaction.

    + Ability to work independently and collaboratively in a team.

    + Proficiency in using EMR systems and mobile health apps.

    Additional Skills & Qualifications

    + Home care experience and familiarity with in-home patient care protocols.

    + CPR/First Aid Certification (preferred or required depending on state regulations).

    + Driver’s license and reliable transportation for traveling between patient homes.

    + Empathy, patience, and cultural sensitivity.

    + Problem-solving skills to adapt care plans to patient needs.

    Work Environment

    Our work environment primarily involves providing care in patients’ homes, requiring adaptability to varied home settings. This travel-based role involves frequent local travel, necessitating reliable transportation and a valid driver’s license. The position is largely independent, with field-based work supported by a remote clinical team. We offer flexible scheduling, including weekdays, occasional weekends, and variable hours based on patient needs. Safety is a priority, with adherence to infection control protocols and PPE usage. Technology-enabled care is integral, using mobile devices for documentation, scheduling, and communication. The role demands physical capabilities such as regular standing, walking, assisting patients with mobility, and occasional lifting of up to 50 lbs.

    Pay and Benefits

    The pay range for this position is $75000.00 - $80000.00/yr.

     

    Paid Time Off & Holidays15 PTO days annually after 90 days, usable for vacation or sick time.Accrual: +1 day per year of service, up to 20 days max.8 paid holidays: New Year’s Day, MLK Day, Memorial Day, Juneteenth, July 4th, Labor Day, Thanksgiving, Christmas.Flexibility: Unused PTO beyond 15 days can be paid out upon request.
