Kind of Work
Social worker would provide intensive case management support & services for individuals with serious and persistent mental illness (primary diagnoses of Schizophrenia and Schizoaffective Disorder). Social worker would be part of a close-knit, multi-disciplinary team comprised of St. Louis County, The Center for Alcohol and Drug Treatment and The Human Development Center.
Essential job duties of T-ACT case management (CM) - often overlap and may be provided concurrently (list is not exhaustive):
• Person-centered mental health treatment planning process – identifying strengths, needs and desires when establishing ongoing treatment goals for mental health recovery.
• Development, assessment, reviewing, and regular updating of mental health treatment plans, functional assessments and LOCUS assessments.
• Coordinating, consulting and maintaining regularly/open communication with team members and natural supports. Natural supports are identified by the client as being important to his/her/their mental health recovery process.
• Monitoring, assessing and restructuring resources, supports and services being utilized by the person in their mental health recovery.
• Frequent consultation & assessment regarding the progress made toward treatment goals and mental health recovery.
• Mental health crisis response services provided to people experiencing increased mental health symptoms.
• Rotating on-call schedule to include nights and weekends.
• Daily assessment of the person receiving T-ACT services for strengths, resources, supports, needs, functioning, health problems and conditions, safety, vulnerability and injury risk. Assessment should include ongoing consultation with natural supports.
• Screening people for substance use and abuse & making appropriate referrals.
• Assisting people with housing/homelessness challenges, identifying resources, assisting with referrals, and helping client access available resources.
• Teaching and implementing mental health recovery skills to reduce mental health symptoms and increase peoples’ overall frustration tolerance and emotional resiliency.
• Regular consultation & coordination with community mental health agencies, housing agencies, medical agencies, chemical dependency agencies, employment agencies, government assistance agencies, social security system, local landlords, and any other agencies the client interfaces with in their lives.
• For people committed as mentally ill by the court to the MN Commissioner of Human Services: social worker will write reports to the court and attend mental health commitment court hearings.

Distinguishing Features of Work
An employee in this class is responsible for providing social work services on cases involving complex social problems and social treatment plans. Duties include performing responsible and difficult casework, directing service programs or providing work direction to paraprofessional staff. Child Protection Services (CPS) Social Workers are assigned social work cases involving voluntary and non-voluntary child protective service cases. The work is performed under the direction of assigned supervisory staff.

SOCIAL WORKER MINIMUM QUALIFICATIONS (Pass/Fail):
Graduation from an accredited college or university with a Bachelor's degree in social work, psychology, sociology or closely related field; OR graduation from an accredited college or university with a Bachelor’s degree in any field AND one (1) year of full-time paid verifiable experience as a social worker in a public or private social services agency.
Possession of a valid driver’s license.
SOCIAL WORKER (MSW) MINIMUM QUALIFICATIONS (Pass/Fail):
Graduation from an accredited college or university with a Master’s degree in social work; OR graduation from an accredited college or university with a Master’s degree in counseling, clinical psychology or a closely related field with a specialization (39 quarter credits or 30 semester credits) in marriage, family, child or victim/offender counseling or a closely related specialization.
Possession of a valid driver’s license.
