    TheSchool Based Early Support Program(SBES) of Graham Windham is a new family support model that focuses on providing support to families whose children are served within the Bronx community school district 8 (Hunts Point) and/or all 3 schools located in Manhattan 4&5 (central and east Harlem). The program enhances partnerships between schools and community providers, offers case management, facilitates parent-support and education groups, and helps families with their basic needs.

    Position Overview

    TheCommunity Pathway Workerwill play a vital role in providing direct support to families within the school community. Using a strengths-based Appreciative Inquiry lens the Community Pathway worker builds strong and productive relationships and uses motivational interviewing techniques to empower families to reach their goals including creating and sustaining thriving environments for their children.

    Key Responsibilities:

    + Family Support and Empowerment:

    + Provide case management services to (8) families in Bronx school district 8 (Hunts Point) and/or in Manhattan 4&5 who voluntarily enroll in the program.

    + Partner with families across 3 schools to develop and implement individualized support plans.

    + Utilize motivational interviewing techniques to support families in identifying their strengths, setting goals, and overcoming challenges.

    + Provide advocacy and connect families with appropriate resources within the community and other supportive systems (housing, public assistance, OPWDD).

    + Conduct Success (Home) Visits to families as needed

    + Complete required documentation on family engagement.

    + Parent Education and Support:

    + Co-facilitate parent support groups alongside the School Outreach Liaison using the evidence-basedTriple P Model,which empowers parents with positive techniques to manage challenging behaviors and build strong relationships with their children.

    + Facilitate open communication and peer support within the parent groups.

    + Attend meetings and trainings at partner schools.

    + Collaboration and Outreach:

    + Work collaboratively with the School Outreach Liaison and other SBES staff to support school partnership and develop strong relationship with families and communities.

    + Assist with school program events and initiatives focused on supporting families within the school community.

    + Work collaboratively with Program Director, Supervisors, Case Planners, and other program staff to organize services.

    + Build a strong and supportive school community:

    + Serve as a trusted liaison between families, Graham programs and community partners

    + Using an Appreciative Inquiry Lens, proactively build trust and rapport with families from diverse backgrounds and promote a respectful, inclusive and solution-focused culture within all 3 schools.

    + Meet with school teams weekly to identify students that can benefit from supports offered by SBES team.

    + Educate school staff about the program and its target population.

    + Keep all stakeholders informed through regular communication, including newsletters, social media, and meetings.

    + Track program participation, outcomes, and data in Graham’s database to maintain accurate records.

    + Implement data-driven improvements and stay up-to-date on best practices in family support and child development.

    + Stay Current on Family Support Best Practices: Attend relevant trainings and workshops to continuously improve program effectiveness. Including but not limited to Appreciative Inquiry, Motivational Interviewing, Trauma informed care, cultural competency and other workforce development trainings.

    + Regularly travels among all 3 schools located in Bronx school district and/or all 3 schools located in Manhattan 4&5 (central and east Harlem).

    Qualifications

    Qualifications:

    + A Bachelor of Arts (BA), Bachelor of Science (BS), or Bachelor of Social Work (BSW) degree

    + Minimum 2 years of experience working with families and children and in partnership with schools.

    Preferred Qualifications:

    + Experience using motivational interviewing techniques in a helping profession.

    + Bilingual English/Spanish preferred.

    + Training in trauma-informed care.

    + Experience using Appreciative Inquiry model.

    + Understanding of child development.

    + Strong communication, interpersonal, and collaboration skills.

    + Ability to build trust and rapport with families from diverse backgrounds.

    + Excellent organizational and time management skills.

    + Possess a strong belief that families have the ability to grow and change.

    + Proficiency in Microsoft Office Suite.

    + Strong understanding of student support services, including IEPs, 504 plans, and SEL programs.

    + Group work skills and experience preferred.

    + Experience working in a school setting, a plus.

    Position Type:

    Full Time Position: Starting Salary from$54,083 - $57,083/annually + Potential to Earn Merit Performance Based Incentive for Eligible Staff + Comprehensive Benefits + Professional Development Opportunities
