    Home Health CWOCN RN

    $10,000 Bonus

    **Position Purpose:** The Home Health CWOCN RN provides expert nursing care in wound, ostomy, and continence management to home care patients. They collaborate with healthcare professionals and patients/families to ensure optimal outcomes.

    Qualifications:

    + Graduate of an accredited nursing program (BSN preferred)

    + Current RN license in the state of practice

    + Certified Wound Ostomy Continence Nurse (CWOCN) certification required

    + Minimum 2 years of RN experience in a clinical setting

    + Proficiency in electronic medical records systems

    + Strong assessment, critical thinking, and communication skills

    + Ability to work independently and as part of a team

    + Valid driver's license and reliable transportation

    + Current CPR/BLS certification

    Essential Functions:

    + Conduct comprehensive assessments of wounds, ostomies, and continence issues

    + Develop individualized care plans with the interdisciplinary team

    + Educate patients and families on wound care, ostomy care, and continence management

    + Provide direct patient care and monitor progress

    + Participate in quality improvement activities and maintain documentation

    + Provide in-service education and participate in continuing education
