    Job Requisition: REQ3869 Employment Type: Full-Time Shift: Days Hours Per Week: 40 hours

     

    Hiring range: $ 18.39-$25.02 The actual compensation for this position will be determined based on experience and other factors permitted by law.

    Responsibilities:

    • Supports parents when a child is first referred to the Youth ACT program

    • Helps parents understand the Youth ACT program so that they can make informed decisions on behalf of their child

    • Ensures that parents understand what to expect, who to contact, and how to be a part of their child's treatment team • Ensures that all Youth ACT assessments include the parent's perspective on their child and take into account the parent's needs for support and assistance

    • Works with the treatment team to ensure that parents have a voice in decisions and understand the treatment options for their child

    • Helps parents identify and set goals and short-term objectives based on their needs and strengths

    • Address barriers that may limit parental participation • Helps parents gather documentation to support transitions

    • Provides family guidance during transitions • Educates parents about community resources and supports

    • Teaches parents about self-care; coping strategies; symptom management • Helps parents recognize and build on their own personal strengths and their child's strengths

    • Helps parents navigate the various systems and resources available to the child and family (mental health; social services; educational; medical)

    • Assists the parents in enhancing family and community connections

    • Participates in all daily ACT team meetings • Documents contacts with family, collaterals, and community resources

    • Provides after-hours on-call support coverage as assigned

    Minimum Qualifications Licensure / Certification:

    Education: High school diploma/GED with 2 years of experience, or Associate's degree, or Technical degree required.

     

    Certified Family Peer Advocate required within one year in position

     

    Work Experience: • Prior related experience preferred
