Job Details Responsibilities include: Provide home-visiting case management services to women in their third trimester and/or families with children aged 0-3. Provide ongoing socio-emotional development and early literacy support Identify and assist clients with accessing preventive and primary health care services as well as essential supports (e.g., pediatric care, Early Intervention, WIC etc.) Conduct basic health assessments, assist families with identifying needs, provide basic health information, and make appropriate referrals through monthly home visits Build, encourage and support partnerships and collaborations with healthcare providers, community based organizations, government agencies, etc. Participate in outreach activities Requirements: High School diploma or GED Knowledge of the community and community resources. Respect for and sensitivity to the needs and rights of others, including those with differing cultural, racial, sexual, or ethnic identities. Good computer skills, knowledge of Microsoft Office Suite (e.g., Word) & Google products Bilingual English-Spanish preferred Valid NYS driver’s license, personal car to carry out employment duties, and active automobile insurance Flexible schedule to accommodate some early morning, evening and weekend events Salary Range USD $22.05 - USD $22.05 /Hr. This range serves as a good faith estimate and actual pay will encompass a number of factors, including a candidate’s qualifications, skills, competencies and experience. The salary range or rate listed does not include any bonuses/incentive, differential pay or other forms of compensation that may be applicable to this job and it does not include the value of benefits.
