    Job Description

    The Clinical Intake Coordinator, LPN facilitates the Home Health & Hospice referral process by receiving referrals from multiple referral sources within and without the Saint Luke's Health System.

     

    _We are seeking a compassionate and dedicated Clinical Intake Coordinator/LPN to join our supportive environment where patients and staff are valued._

     

    + Monday – Friday, 9 a.m.-5:30 p.m. One remote on-call shift required every 5-6 weeks; workdays will flex to maintain a 40-hour work week. Rotating (remote) holiday requirement.

    + Hybrid - Employees are required to work on-site 3–4 days per week, with the option to work remotely 1–2 days per week. Remote days are scheduled on a rotating basis to ensure adequate office coverage. Work arrangements are subject to change based on business needs.

    + Determines need for additional information from referral sources

    + Communicates agency acceptance or decline of referral to varied referral sources, and processes accepted referrals.

    + Missouri and Kansas LPN License Required.
