    Title: Nurse Care Manager for Drug User Health Services

     

    Salary: $110,000 - $120,000

    Program Overview:

    Project Renewal provides low-threshold health care services for people who use drugs and/or struggle with addiction. This care includes the provision of buprenorphine maintenance treatment and other MOUD for our clients with opioid use disorder via our primary care and psychiatry clinics and our mobile medical vans and referrals to external providers. This treatment is offered in a patient-centered manner, supporting patient goals that range from managed or safer use to complete abstinence. This treatment is also offered alongside comprehensive drug user health services, which include routine primary and psychiatric care, overdose prevention, and treatment of HIV and Hepatitis C infection.

    Position Overview:

    Project Renewal is seeking a Nurse Care Manager (NCM) for Drug User Health Services that would coordinate care for clients seeking or receiving buprenorphine and/or other drug user health services at PRI. The Nurse Care Managers are key members of the buprenorphine treatment team. The Nurse Care Manager would report to the Nurse Care Manager for Drug User Health and the Medical Director of Addiction Medicine & Drug User Health.

     

    The ideal candidate must possess not only the clinical knowledge and skills to participate in individual patient treatment, but also the organizational and communication skills to execute systems level activities. The NCM is the point person that will have the most contact with patients receiving buprenorphine or other drug user health services, will be in continued contact with the prescribing providers to communicate patient’s needs and shape patients’ care, and will coordinate linkages with other programs within and outside the organization. This position includes the option of working remotely for part of the week.

    Essential Duties & Responsibilities:

    + Seeing patients daily, participating in patient assessment and preparation, including day-to-day program concerns, education, and counseling

    + Enrolling patients into the buprenorphine treatment program (or other forms of office-based addiction treatment, such as medication for alcohol use disorder), including initial screening & assessment of patients

    + Supporting the patient and prescriber in buprenorphine initiation, stabilization, and maintenance treatment procedures under the supervision of the prescribing provider

    + Assisting members of the health care team in the formulation and implementation of the plan of care

    + Assisting the prescribing provider in making referrals to community providers for counseling or higher levels of care when needed

    + Maintaining therapeutic relationships with both the patients and the medical providers

    + Overdose prevention counseling and training in the use of naloxone.

    + Overseeing the following patient care components:

    + Care coordination including medication management and treatment monitoring

    + Review of prescription monitoring program to track prescribing and pharmacy fill history

    + Insurance authorization and troubleshooting coverage issues

    + Relationship building and patient linkage to additional support (including low barrier services, mental health care, and formal substance use treatment services if necessary)

    + Facilitation of ancillary services (including patient transportation)

    + Sharing information with other members of the health care team and evaluation team through chart documentation, interdisciplinary team meetings and email communication

    + Record keeping that may be referenced for a DEA inspection (managing the buprenorphine census)

    Qualifications:

    + Active New York State Registered Nurse (RN) license

    + BLS certification

    + At least 2 years of experience working with populations with high poverty, chronic illness and/or behavioral health issues

    + Knowledge of low-threshold and trauma-informed care and motivational interviewing techniques

    + Prior experience conducting individual patient education and counselling sessions.

    + Demonstrated ability to work collaboratively in a team environment.

    + Demonstrated computer literacy in Microsoft and web-based applications.

    + Excellent verbal and written communication skills.

    + Excellent interpersonal and organizational skills, including problem solving with a team.

    + Knowledge of community resources; demonstrated ability to network and build strong relationships with community organizations serving priority populations as identified by the agency and/ or funder.

    + Demonstrated ability to work with patients of diverse backgrounds, underserved communities, and co-morbidities

    + Please note that this position requires significant walking and/or standing in shelter and community.

    + Remote work for part of the regular schedule is available.

    + Some weekend availability preferred.

    Preferred Skills:

    + Ability to speak and write in Spanish

    + Prior clinical experience working with patients with substance use disorders and mental health diagnoses.
