    Responsibilities

    The Behavioral Health Care Manager is a core member of the collaborative care team, including the patient’s medical provider and psychiatric consultant, as well as the larger primary care team or medical team. The behavioral health care manager is responsible for supporting and coordinating the mental and physical health care of patients on an assigned patient caseload with the patient’s medical provider and, when appropriate, other mental health providers.

    Qualifications

    MINIMUM

    Master's degree in either Counseling or Social Work

    2 + years experience as a mental health counselor in a behavioral health setting

     

    Licensed Marriage and Family Therapist (LMFT), Licensed Clinical Social Work (LCSW), Licensed Masters of Social Work (LMSW) and / or Licensed Professional Counselor (LPC)

     

    American Heart Association BLS

    PREFERRED

    Licensed Marriage and Family Therapist OR Licensed Social Worker OR Nurse Practitioner OR Licensed Psychologist

     

    Experience providing brief psychosocial interventions such as problem-solving treatment

     

    Experience with evidence-based counseling techniques

    Pay Range

    $35.62 - $52.99 /hour
