Description
Senior Community Service Assistant (SisterFriend)
Under general supervision, the SisterFriend – Senior Community Service Assistant (SCSA) is the community's primary link to the Detroit Health Department and SisterFriends. The SCSA be a part of a larger team responsible for recruiting, supporting, and working with volunteer mentors and sisters who are pregnant. The SCSA reaches out to underserved families; assist in the enrollment into appropriate programs; provides health and social support, information and referrals; provide advocacy for families with agencies, programs, and other entities; represents program at community functions; assists with service provision to individuals, families, and groups; facilitate recruitment of community, businesses and local partners.
Examples of Duties
+ Provides case management services to clients, including assessment, linking/coordinating, monitoring, and reassessment.
+ Plans, schedules and participates in interviews of program participants to obtain
+ Conduct background information, conduct assessments, and determine eligibility and the nature and extent of services required.
+ Inform participants on program requirements and services.
+ Determines the most effective method for providing service to non-routine participants or those with special requirements and explains available options.
+ Refers participants to other agencies or programs for specialized services.
+ Follows up on referrals and maintains data on the numbers of clients referred.
+ Evaluates the effectiveness of programs and makes recommendations for improvements.
+ Ensures that programs conform to applicable federal regulations and departmental policies.
+ Provides input on policies or procedures designed to achieve the objectives of the assigned agency.
+ Develops and implements training educational training sessions to participants.
+ Conduct community outreach activities.
+ Performs community outreach to inform the public and other service providers of agency objectives and activities.
+ Maintains contacts with public and private social agencies to obtain information referrals.
+ Provide case management services to expectant mothers up until the baby is one, including assessment, linking/coordinating, monitoring, and reassessment
+ Coordinating with program's mentor matched with pregnant women and mothers on my caseload to decrease social isolation and promote relationship between the mentor and mother
+ Acts as an “information link” to the community and program organizations by keeping up to date on community resources and events that could benefit individuals and families.
+ Recruit and enroll women for the SisterFriends program.
+ Conduct appropriate screening for health risk and program eligibility.
+ Work closely with division and departmental teams, community partners, and health system to develop SisterFriends, little sister and family resources.
+ Provide out formal and informal presentations on program services.
+ Maintains complete and accurate records of activities and services provided.
+ Work in Partnership with other Maternal Child Health Programs and assist with community outreach activities.
+ Provide weekly dashboard and monthly activities reports for fathers forward program with related metrics and activities
+ Participates in team reviews, staff meetings and staff trainings.
+ Provide education and enrichment trainings for participants.
+ Engage program participants
+ Help SisterFriends and little sisters’ complete reproductive life plans, that include education and information to achieve the best possible health before birth, between pregnancies and after delivery.
+ Assist with media recruitment.
+ Provide supportive services via home visits. Contributes to building positive relationships.
+ Educates SisterFriends volunteers about navigating the health care environment to assure families receive needed benefits and services.
+ Mentor women and families in the development of positive parenting practices and coping mechanisms.
+ Provide transportation services to program participants
+ Suggest approaches for disseminating public health data and information to the public
+ Other Duties as Assigned
Minimum Qualifications
Bachelor’s degree in the social sciences, education, psychology, guidance and counseling or related field.
One (1) year of recent experience as a supervisor, preferably in a social, economic or community development program.
Three (3) years of experience in case work, group work, individual and group counseling and guidance, social research or community or public relations.
Equivalent combinations of education and experience may be substituted to meet the education and experience requirements of this position.
Reliable transportation
Minimum of one-year work experience in a health or human service setting; Preferred two-three years of experience in human services, case management and/or working with clients to provide education, identify support needs, develop service plans, and problem solving
Experience working with health care systems and community-based organizations
Experience working with multicultural communities
Ability to work evenings and weekends as needed.

RETIREMENT BENEFITS
City Employees Retirement System
As a regular City employee you automatically become a member of the General Retirement System. This entitles you to a retirement allowance after:
• Completion of thirty (30) years of service;
- At age sixty (60) if you have at least ten (10) years of service, or
- At age sixty-five (65) with eight (8) years of service.
