Registered Nurse Assessment Coordinator
Type: Full-Time, Permanent
Overview:
Seeking a Registered Nurse to conduct in-home and virtual assessments for health plan eligibility and care planning. This role supports member independence and long-term health through person-centered service plans.
Key Responsibilities:
+ Perform UAS-NY assessments and evaluate clinical, behavioral, and social needs
+ Develop care plans and educate members on benefits and services
+ Coordinate with care teams and maintain accurate documentation
+ Ensure compliance with DOH and MLTC regulations
Qualifications:
+ RN licensed in NYS; Associate degree required, BSN preferred
+ 1+ year nursing experience preferred
+ MUST have experience with UAS-NY and long-term care
+ Strong communication and EMR skills
+ Valid NYS driver’s license and reliable transportation to conduct site visits

Pay and Benefits
The pay range for this position is $90000.00 - $100000.00/yr.
