    Position Program Supervisor, SP-SRO

     

    Full-Time/Part-Time Full-Time

     

    Location Amityville, NY

     

    Salary $65,000

     

    Days/Hours Tuesday - Saturday; 8am - 4pm

    Description

    Concern Housing is seeking a Program Supervisor to be responsible for the overall supervision and management of program site to help promote tenant growth, safety, and the effective delivery of support services.

    Duties and Responsibilities:

    + Assist with the supervision and coordination of all direct care services, including staff scheduling, supervision and crisis intervention where needed (including 24/7 cell phone coverage).

    + Assists Program Director with maintaining program occupancy rates including, but not limited to, collection of supporting documentation and admission process.

    + Oversight of the Liberty Village driver, who is an employee of Beacon House.

    + Provide case management services (service planning, medication monitoring, assistance in forming linkages and/or referrals to clinical services, day programs, vocational services and opportunities, community integration, entitlements etc.)

    + Responsible for all documentation and timely submission of work.

    + Housing services, including assistance with maintenance issues.

    + Provide monthly unit inspections and other site visits as per program requirements, and as needed or directed by Director.

    + Compliance with LIHTC regulations, including eligibility determination, annual certifications and any other reporting requirements, as necessary.

    + Monitoring client charts for timely submission of paperwork, and qualitative review of progress notes, services plans and reviews, functional assessments and other required documentation.

    + Cooperation with the Property Management department re: site maintenance and safety issues, as well as the accounting department to ensure monitoring of rental payment/ arrears and assisting special needs tenants with securing and maintaining entitlements.

    + Provide staff training and development to ensure suitable implementation and evaluation of evidence-based best practices. Evaluate staff performance and promote professional growth

    + Conduct continuous needs assessment for program development and evaluation and quality assurance. Closely monitor health care outcomes of special needs tenants.

    + Collaboration with local hospitals and healthcare/ care coordination providers through committees, special projects, etc., as well as community outreach.

    + Remain current with all Agency required compliance documents and trainings.

    + Other tasks as assigned by the Program Director

     

    Salary: $65,000

    Position Requirements

    Master’s Degree in Psychology, Social Work or related mental health field with at least 3 years experience working in a similar setting and/or working with people with mental illnesses. The individual in this position must have good leadership and supervisory skills, good verbal and written communication skills and the ability to make sound judgments regarding client care.
