MercyOne Home Care – Des Moines is seeking a compassionate and dedicated **Registered Nurse (RN)** to join our home health team. In this role, you’ll provide high-quality, patient-centered care in the comfort of patients’ homes, helping them maintain their health, independence, and dignity.

Key Responsibilities
+ Conduct comprehensive assessments of patients’ health status and needs
+ Develop and implement individualized care plans in collaboration with interdisciplinary teams
+ Administer medications and treatments, monitor patient responses
+ Educate patients and families on disease management and health maintenance
+ Monitor vital signs and manage chronic conditions
+ Accurately document patient care activities
+ Coordinate care with physicians, therapists, and other healthcare professionals
+ Provide emotional support and counseling to patients and families
+ Ensure compliance with healthcare regulations and standards
+ Participate in quality improvement initiatives and ongoing education
Qualifications
+ Current RN licensure in the state of Iowa
+ BSN preferred; ADN accepted
+ Previous home health or community nursing experience preferred
+ Strong clinical assessment and critical thinking skills
+ Ability to work independently and manage time effectively
+ Valid driver’s license and reliable transportation
