    As a Hospice LPN You will:

    + Give hands-on care to patients nearing the end of life, following state nursing rules .

    + Follow the care plan after assessing the patient’s needs.

    + Keep clear and timely records of the care provided.

    + Teach and support patients and their families about the illness and care plan.

    + Help coordinate care by communicating with the hospice team (like aides, chaplains , social workers).

    + Manage pain and symptoms, checking how well treatments are working .

    + Take part in training and quality improvement activities to grow professionally and improve care.

    Qualifications

    + Graduate of an accredited practical nurse or vocational nursing program.

    + Three years nursing experience. Community health/hospice or medical/surgical experience is preferred.

    + Currently licensed as an LPN/LVN in the State.

    + Comply with accepted professional standards and practice.

    + Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order.

    + Possess and maintain current CPR certification.

    + Understand philosophy of hospice concept . Also understand needs of the terminally ill.

    + Use own knowledge and experience base and other resources as necessary to make logical decisions and solve problems.

    + Demonstrate good verbal and written communication, and organization skills.
