Job Purpose:
The primary function of this role is to assist in the facilitation of the AM & PM activities of the Structured Day program.
Essential Functions:
+ Provide protective oversight and supervision for individuals assigned.
+ Assist participants with daily goals, including eating, mobility, and personal care needs (hygiene, bathing, grooming, dressing, toileting, eating).
+ Collaborate with team in addressing sudden changes and/or emergencies that may arise.
+ Follow the highest safety standards and promote workplace safety.
+ Communicate significant participant changes or challenges with Supervisor, family/caregivers, and Service Coordinators.
+ Maintain the physical space to ensure the health, safety, and well-being of participants.
+ Ensure shift has proper staffing coverage; provide direct coverage as needed to maintain required support levels.
+ Monitor all program and funder-required documentation; follow up on employee documentation to ensure accuracy and timeliness.
+ Ensure all required reports are completed per specification.
+ Report all policy violations to appropriate personnel.
+ Organize on-site and off-site community activities and events, including trips and volunteer opportunities.
+ Utilize existing community resources to improve program and encourage participant engagement.
+ Collaborate with team in implementing and orchestrating the day’s activities and communicating any modifications needed, ensuring a quality program is offered daily.
+ Facilitates participant group discussions and activities and participates in community trips.
+ Provides assistance and instruction services daily to achieve the participant’s goals and objectives, provide support and assistance to Direct Support Professionals (DSPs).
+ Provides protective oversight and supervision for individuals assigned.
+ Works with the participant’s support team to continually assess needs, progress, and barriers.
+ Maintains the health and well- being of people supported.
+ Plans activities and events within established budget.
+ Develops a monthly calendar /program of activities that is person -centered and therapeutic in nature, such as art, exercise, music, activities involving hand-to-eye coordination, easy math, word games both individual and group, sensory activities, such as cooking/food, aromatherapy, and tactile activities such as gardening, clay, sand.
+ Participates in On-Call rotation as scheduled.
+ Collaborates with team in addressing any sudden changes and/or emergencies that may arise.
+ Follow up on team documentation to ensure its accurate and up to date.
+ Ensure shift has proper levels of coverage; provide direct coverage as needed to ensure proper levels of support.
+ Provides specific training to DSPs as requested/required.
+ Ensures all required reports are completed per specification.
+ Promotes workplace safety.
+ Performs other duties as assigned by the Structured Day Program Supervisor and/or designee within scope of practice.
Non-Essential Functions:
+ Participates on various internal committees as able.
+ Attends all meetings as required.
Compliance:
+ Must maintain confidentiality and exercise discretion concerning all business-related matters, including but not limited to board/vendors/donors’ names, investigations, finance, operations, and employee relations matters.
+ Must be willing to complete specific company required trainings as mandated by our governing agency, Office for People with Developmental Disabilities (OPWDD), at time of hire and annually thereafter
+ Must adhere to all company policies & department procedures at all times & ensure compliance with direct report.
Education and Experience:
Minimum High School Degree or GED with five (5) years of experience in human service field & three (3) years supervisory experience. Associate’s degree preferred with one (1) year experience in the human service field & one (1) year supervisory experience.
Computer:
Strong working knowledge of Microsoft programs to include Email, Word, Excel, PowerPoint.
Qualifications:
+ Must possess and maintain a valid NYS Driver’s License. A valid license is required to operate company vehicles.
+ Mature interpersonal/professional skills to effectively collaborate, resolve issues, and maintain confidentiality.
+ Good verbal and written communication skills.
+ Demonstrated ability to use poise, tact, and diplomacy, during difficult and/or stressful situations.
+ Proven ability to work in a fast-paced environment, with demonstrated ability to manage multiple tasks, and customer requests, while meeting deadlines.
Supervision:
+ Oversight of approximately six (6) Direct Staff Professionals (DSPs), to include hiring, new hire orientation, yearly training, work schedules, and performance management.
+ Maintains required staffing levels of supervision of participants.

+ Must be able to talk, hear, stand, walk, and/or sit for extended periods of time; may have to twist, bend, use hand to finger, stretch with hands and arms.
+ Specific vision abilities required by this job include close vision and ability to adjust focus.
+ May be exposed to strong odors from time to time.
+ Must be able to lift and carry up to (50) lbs.
+ While performing the duties of this position, exposure to changing weather conditions will occur.
