+ Reviews discharge medication list for accuracy.
+ Processes discharge prescriptions.
+ Accurately maintains required documentation.
+ Maintains a clean and organized environment.
+ Communicates effectively with interdisciplinary team members.
+ Provides patient education on discharge medications.
+ Precepts pharmacy students and pharmacy residents.
Education, Training & Experience :
+ Advanced Pharmacy Degree (Pharm.D.)
+ Board Certification preferred in the first year of eligibility.
+ One to two years’ experience in ASHP accredited residency or equivalent experience preferred.
+ Licensed or eligible for licensure in the State of Indiana.
+ Must take licensure examination at the earliest convenience with licensure received within 75 days following first day of employment in position.
Job duties of a graduate pharmacist may be performed under the supervision of a pharmacist. Failure to pass the licensure exam results in a change of status to Pharmacy Technician/Pharmacy Intern.
