Summary:
+ The Acute Rehab Social Worker acts as the over-all care coordinator of each person served, family/support system and collaborates with the multi-disciplinary team in order to achieve the desired outcomes defined in the individualized plan of care built by the rehabilitation physician. The social worker is knowledgeable about the acute rehabilitation program and is identified by the person served, the family and support system. The social worker perform activities to meet the needs of the persons served in a manner that optimizes the discharge planning process and facilitates post-acute rehabilitation continuity of care. The social worker undertakes a leadership role in advocacy, negotiation, mediation and brokering for the rights and entitlements of the persons served. The social worker provides counseling, education and support programs that enhance the ability of persons served/family to cope with physical, emotional, cultural, religious, lifestyle and environmental changes/concerns.
Responsibilities:
1. Demonstrates knowledge of age related issues that pertain to addiction, illness/disability, domestic abuse, coping, care-giving, bereavement and other psychosocial pertinent issues that impact on patients and families.
2. Available to interact with the person served, the team of the person served, the family/support system and other stakeholders.
3. Facilitates orientation for the person served and is responsible for ensuring communication with internal and external resources.
4. Brings forward to the interdisciplinary team the available financial information to facilitate decision making about the following processes on intake, assessment, service planning, service provision, discharge/transition planning and long-term follow up.
5. Facilitates the involvement of the person served throughout the rehabilitation process.
6. Facilitates the gathering of information to assist the organization in follow-up activities for its analysis of program performance.
7. Ensures that discharge/transition recommendations are completed.
8. Ensures that discharge/transition recommendations are communicated to appropriate stakeholders.
9. Responds to patients and families in a timely manner according to standards.
10. Develops and maintain contacts with community agencies to gather information on available resources and share information with the interdisciplinary team and person served on an ongoing basis.
11. Remains current in government and community service programs, such as, Medicare, Medicaid, PAAD, The Joint Commission, DOH, CARF and other pertinent standards and regulations on ongoing basis.
12. Adheres to department and medical center policy and procedures.
13. Assesses the persons-served/ families’ psychosocial needs according to standards.
14. Develops and implements treatment goals and appropriate services for comprehensive discharge planning, psychosocial intervention, crisis intervention, etc.
15. Performs ongoing reassessment of persons-served/ family’s needs.
16. Performs financial assessments and assists persons served in securing entitlements, programs, etc. that address their needs.
17. Responds to the needs and concerns of persons served in a timely manner according to standards.
18. Provides timely and proper follow-through on treatment goals.
19. Provides appropriate emotional support, counseling and bereavement services.
20. Provides age appropriate education to persons-served/families on psychosocial issues, entitlements, community resources and coordinate vocational and/or tutorial services for patients and families as indicated.
21. Integrates brokering, negotiating, mediatory and advocacy skills into daily work regime.
22. Consistently completes and expedites appropriate paperwork to ensure timely service.
23. Collaborates with persons-served, community agencies and health care team members on an on-going basis and provides update information.
24. Coordinates services with interdisciplinary team members.
25. Regularly attends the weekly interdisciplinary team meeting.
26. Communicates verbal and written information in a timely manner both laterally and vertically to peers and supervisors.
27. Records social work assessments, progress notes, discharge summaries in an objective, clear and concise manner at all times in patients’ medical records in accordance with appropriate timelines.
28. Documents reassessments of the needs of the persons served.
29. Maintains appropriate work statistics, reports and summaries, and submits them to appropriate personnel in a timely manner.
30. Conducts and/or facilitates support groups, lectures, workshops, etc. to the community and/or health care professionals, as assigned.
31. Updates and obtain insurance authorization of persons served and assist in the filing of appeals in cases of insurance denials.
32. Supervises Social Work students from affiliated universities, upon request.
33. Participates in performance improvement, in-services, supervision, department, team, and committee meetings as assigned.
34. Adheres to NASW standards and ethics at all times.
35. Serves as consultant and resource to hospital staff and community, on an ongoing basis.
Credentials:
+ LCSW or LSW (Required)
Education or Equivalent Experience:
+ Masters of Social Work from graduate school of social work approved by the Council on Social Work Education required.

Salary Range:
$64,390.56 - $102,664.22/Annually based on 40 hours per week
