Advanced Practice Professional (Nurse Practitioner) Critical Care Medicine Service

A total of four (4), 1.0 FTE, Advanced Practice Professionals (either NP or PA) will be hired to fill this job opening.
Job Summary
The Critical Care Medicine (MCM) service is a team of attending physicians, house officers, and advanced practice providers (APPs) that provide care to an array of medically complex patient populations at the intensive care acuity level. The MCM team cares for patients with a wide variety of underlying medical conditions who have developed cardiopulmonary instability or multiorgan injury/failure requiring an intensive care unit (ICU). The MCM team comprised of attending physicians and house officers is well established within the institution and is now expanding to include advanced practice providers that deliver high-quality and evidence-based care as primary providers under the supervision of a pulmonary critical care attending.

**Full Time Salary range:** $134,723 - $207,024 / year

Responsibilities*
Duties will include but are not limited to:
+ Perform initial assessments of ICU patients.
+ Perform detailed history and physical, formulate initial assessment and plan, and present patient cases and any pertinent data to medical staff.
+ Provide continuing care of ICU patients admitted to the MCM service, and manage hospital discharge or transfer to other services when appropriate.
+ Appropriately order, collect, and interpret reports from diagnostic lab testing, radiographic procedures, and other clinical studies.
+ Prescribe medications and enter other orders related to patient care.
+ Perform various diagnostic, therapeutic, and procedural clinical interventions, based on privileges such as central lines, arterial lines, chest tube placement and removals, tracheostomy removal, decisions to extubate patients, thoracentesis, paracentesis, bedside ultrasound
+ Manage mechanical ventilation, non-invasive ventilation, heated hi-flow nasal canula for respiratory failure
+ Manage hypotension with fluids, blood products, albumin, vasopressors
+ Manage antibiotics and antifungals
+ Manage gastrointestinal bleeding and liver failure patients
+ Manage renal failure patients requiring dialysis and continuous renal replacement therapy
+ Manage immunosuppression and opportunistic infections in transplant and immunosuppressed patients.
+ Manage chest tubes, including decision to place, administering thrombolytics, placement on suction or water seal or clamp, decision to remove chest tube.
+ Run ACLS codes for cardiac arrest
+ DC cardioversions for cardiac arrhythmias with hemodynamics instability
+ Initiate transplant evaluations for end stage lung and end stage liver patients
+ Assist attending physicians in orientation of new medical trainees to the service, such as resident physicians or medical students; assist in the training of new advanced practice providers to the service; assist in maintaining continuity of care throughout the hospital stay
+ Communicate appropriately with attending physicians, consultants, and other members of the patient care team in order to coordinate and provide clinical care.
+ Maintain accurate and thorough documentation regarding patient status, evaluations, and treatments.
+ Streamline the patient transfer process between levels of care, including writing discharge orders and summaries and communicating specific patient needs to internal and external care centers or team members, as well as ambulatory care providers as needed.
+ Participate in quality assurance activities including quality improvement projects, peer evaluations and support research initiatives involving the ICU patients.
+ Assist in the development of patient-specific, facility-based disease management protocols; perform other related duties as required to assure patient safety and satisfaction; and utilize new improvements and/or technologies that relate to job assignments
Organizational Relationships and Practice Environment:
? Advanced Practice Professionals (APPs) will receive medical supervision from the Critical Care Medicine Service Chief or designee.
? Advanced Practice Professionals (APPs) will receive administrative supervision from the APP Manager.

Required qualifications must be met by the candidate in order to be interviewed and considered for the position.

Required Qualifications*
NP:
+ Current licensure to practice as a Registered Nurse and Nurse Practitioner specialty certification in the State of Michigan.
+ Current specialty certification to practice as an Adult-Gerontology Acute Care Nurse Practitioner **or**
+ Current specialty certification to practice as a FNP, AGPCNP, or ANP with 4 years experience as an APP in the ICU clinical environment with the same acuity and patient population-within the past 6 years.
+ Graduate of an accredited Nurse Practitioner program.
+ Certification as a Nurse Practitioner recognized by the National Council of State Boards of Nursing (NCSBN).
+ Current DEA registration required
+ ACLS required (separate AHA approved BLS also required unless ACLS was completed through the UMHS Simulation Center)
+ One (1 year) or more of clinical ICU experience within the last 5 years caring for acutely ill patients.
PA:
+ Current license to practice as a Physician Assistant in the State of Michigan.
+ Satisfactory completion of a Physician Assistant program accredited by Accreditation Review Commission on Education for the Physician Assistant (ARC-PA) or one of its predecessor agencies, CAHEA or CAAHEP.
+ Certification by the National Commission on Certification for Physician Assistants (NCCPA).
+ State of Michigan Controlled Substance License Required
+ Current DEA registration required
+ ACLS required (separate AHA approved BLS also required unless ACLS was completed through the UMHS Simulation Center)
+ One (1 year) or more of clinical ICU experience within the last 5 years caring for acutely ill patients

Desired Qualifications*
+ Demonstrated expertise in procedures (i.e. central line placement; thoracentesis)
+ Experience rounding on an inpatient service for a minimum of 8 months and caring for acutely ill patients
+ APP experience with Critical Care Medicine Patients
+ Demonstrated ability to coordinate clinical care ideas across multiple patient care services and to communicate as a patient advocate.
+ Ability to prioritize assignments and manage a busy work environment
Work Schedule
Hours: 40 hours per week
Shift: Weekday and Weekend days and nights rotating as needed
