    Job Summary and Qualifications

     

    The Burn Care Navigator’s primary function is to build a relationship with patients and physicians, to coordinate a plan of care including appointments, transportation, education, provision and / or enablement of support services and representation within the multidisciplinary care environment.

    In this role you will:

    + **Function as a member of the multidisciplinary team as an advocate, educator, and liaison for burn and wound patients.**

    + **Be responsible for ensuring all patients, with a burn or wound diagnosis receive quality and comprehensive services by coordinating patient care through the continuum in collaboration with the multidisciplinary team serving as a clinical resource with expertise in burn and wound care management.**

    + **Serve as a liaison throughout the facility and in the community regarding services provided for this unique patient population.**

    + **Provide expert nursing care, which includes; direct clinical practices, consultation, education, and management of burn/wound related calls and messages.**

    + **Assist patients with referral appointments, schedule ordered tests, and order related wound care supplies/devices.**

    + **Triage new patients to assess and prioritize the acuity and order of care and assist with patient flow between the wound center, lab, x-ray, surgery, ICU, and other departments.**

    + **Assume responsibility and accountability for the management of resources to achieve efficient, high quality outcomes for each burn patient including support for interdisciplinary and cross facility collaboration ie SOAR monthly meetings, weekly grand rounds, attendance at monthly peer review, and bimonthly Burn committee meetings.**

    What qualifications you will need:

    + **Bachelors of Science**

    + **Registered Nurse Idaho or Compact**

    + **Successful completion of the certification requirements for Wound, Ostomy, Continence Nursing or Certified Wound Specialist is preferred within 3 years of hire.**

    + **Minimum of 2 years of previous related experience**

    + **ALS or ACLS must be obtained within 6 months of hire.**

    + **Procedural Sedation Certification must be obtained within 2 months of hire**
