What You’ll Do

+ Deliver skilled nursing care to patients in their homes, following physician orders and individualized care plans.

+ Conduct initial assessments and collaborate with Population Health Case Managers.

+ Supervise and support LPNs and Home Health Aides.

+ Focus on achieving excellent patient outcomes and satisfaction.

What You’ll Need

+ Graduate of an accredited nursing program.

+ Active **RN license in Delaware** .

+ Minimum **1 year of nursing experience** (Home Care or Acute Care preferred).

+ Valid **driver’s license** and reliable transportation.

+ Willingness to work **1 weekend/month** and **2 holidays/year** (1 summer, 1 winter).
